| [目的]总结名老中医程锦国基于“肾络”学说的认识,对局灶节段局灶节段性肾小球硬化(focal segmental glomerulosclerosis,FSGS)在分子细胞层面进行新的诠释,并阐述该病的发生机制、发展转归、辨病辨治、临证经验的认识。[方法]通过临床跟师、收集病例,查阅相关古籍文献,对程锦国主任中医师运用肾络理论探讨FSGS进行分析及总结,并举以临床医案验证体现。[结果]程师认为,肾小球形态、功能上与络脉类似,其中肾络空虚是肾络病的发病基础,肾络瘀痹为其主要病理改变,风湿热邪入络化瘀是疾病进展的关键环节,治疗上多以补肾健脾、活血化瘀、解毒清热、祛湿化浊为要,并取得了良好的临床疗效。[结论]名中医程锦国将肾络理论与FSGS临床相结合,在控制疾病进展、改善肾功能、减轻症状上疗效显著,为FSGS的中医药治疗提供新的思路。 |

| [objective]To summarize the understanding of TCM CHENG Jinguo"s Experience Based on the theory of “Renal collaterals”, and give a new interpretation of focal segmental glomerulosclerosis (FSGS) at the molecular and cellular levels. The pathogenesis, development and outcome of the disease, treatment based on syndrome differentiation, and the recognition of clinical experience were expounded. [Methods] Through following and consulting teachers’ clinical works, collecting cases and referring to relevant ancient books and literatures, TCM CHENG Jinguo"s experience in treating FSGS based renal collateral theory was analyzed and summarized, and one clinical case was presented. [Results] In the view of TCM CHENG Jinguo, the glomerulus is similar to the collaterals in form and function, in which the kidney collateral emptiness is the basis of the basis of kidney collateral disease, the kidney collateral stasis and obstruction is the main pathological change, the pathogenic factors like wind, damp and heat enters the collaterals to form stasis is the key steps of the disease progression, and the treatment is mainly to tonify the kidney and spleen, promote blood circulation and remove stasis, detoxify heat, remove dampness and turbidity. The clinical effect is significant. [Conclusions] TCM CHENG Jinguo combined renal collateral theory with FSGS clinical practices, which has a significant effect on controlling disease progression, improving renal function and alleviating symptoms, providing a new idea for TCM treatment of FSGS. |
