      目的:探究多囊卵巢综合征患者内分泌相关指标及其与心血管疾病危险因素的相关性。方法:采集2018年1月~2020年6月在我院内分泌科进行治疗的多囊卵巢综合征患者120例作为实验组,同时采集同阶段在我院进行体检的健康人群61例作为对照组,两组实验对象均行多普勒超声检测及阴道二维超声检测,并比较两组实验对象阻力指数(RI)、子宫及卵巢间质动脉搏动指数(PI)、卵巢体积(2D-OV)、卵泡数(FN)。取两组实验对象静脉血并分离血清,比较两组实验对象心血管疾病危险因素[低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FPG)、总胆固醇(T-cho)、甘油三酯(TG)、糖化血红蛋白、C反应蛋白(CRP)水平]、内分泌相关指标[25-羟维生素D(25-OH-VitD3)、卵泡生成激素(FSH)、黄体生成激素(LH)、Betatrophin]水平,并进一步分析多囊卵巢综合征患者彩色多普勒超声参数、二维超声参数、内分泌相关指标与心血管疾病危险因素相关性。结果:实验组多囊卵巢综合征患者2D-OV、FN、子宫PI均较对照组显著升高,RI、卵巢PI均较对照组显著降低(P<0.01)。实验组多囊卵巢综合征患者糖化血红蛋白、LDL-C、CRP 、FPG、SBP、TG水平均较对照组显著升高,HDL-C水平均较对照组显著降低(P<0.05)。两组DBP、T-cho水平相比差异无显著性(P>0.05)。实验组多囊卵巢综合征患者25-OH-VitD3 水平较对照组显著降低,Betatrophin 、LH水平均较对照组显著升高(P<0.05)。25-OH-VitD3、RI、卵巢PI均与HDL-C呈正相关,与BMI、LDL-C、FINS呈负相关(P<0.05)。25-OH-VitD3、RI、卵巢PI均与HDL-C呈正相关,与糖化血红蛋白、LDL-C、CRP、FPG、SBP、TG呈负相关(P<0.05)。Betatrophin、FSH、LH、2D-OV、FN、子宫PI均与糖化血红蛋白、LDL-C、CRP、FPG、SBP、TG呈正相关,与HDL-C呈负相关(P<0.05)结论:多囊卵巢综合征患者血清中25-OH-VitD3水平显著降低,Betatrophin 、FSH 、LH水平显著升高,且25-OH-VitD3与HDL-C呈正相关,与BMI、LDL-C、FINS呈负相关,Betatrophin、FSH、LH与BMI、LDL-C、FINS呈正相关,与HDL-C呈负相关。

      Objective: To explore the correlation between endocrine related indexes and cardiovascular disease risk factors in patients with polycystic ovary syndrome (PCOS). Methods: 120 patients with polycystic ovary syndrome treated in the Department of endocrinology of our hospital from January 2018 to June 2020 were selected as the experimental group, and 61 healthy people who had physical examination in our hospital at the same stage were selected as the control group PI, 2d-ov and FN were measured. Blood samples were collected from venous blood of two groups and serum was separated. The risk factors of cardiovascular disease [low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), fasting blood glucose (FPG), total cholesterol (T-Cho), triglyceride (TG), glycosylated hemoglobin (HbA1c), C-reactive protein (CRP) levels], endocrine related indicators [25 hydroxyvitamin D] were compared between the two groups(25-oh-vitd3), follicle forming hormone (FSH), luteinizing hormone (LH) and betatrophin] levels, and further analyzed the correlation between color Doppler ultrasound parameters, two-dimensional ultrasound parameters, endocrine related indicators and cardiovascular disease risk factors in patients with polycystic ovary syndrome. Results: compared with the control group, 2d-ov, FN and Pi of uterus in experimental group were significantly increased, RI and Pi of ovary were significantly decreased (P < 0.01). The levels of glycosylated hemoglobin, LDL-C, CRP, FPG, SBP and TG in the experimental group were significantly higher than those in the control group, while the HDL-C level in the experimental group was significantly lower than that in the control group (P < 0.05). There was no significant difference in DBP and T-Cho levels between the two groups (P > 0.05). The level of 25-oh-vitd3 in the experimental group was significantly lower than that in the control group, and the levels of betatrophin, FSH and LH in the experimental group were significantly higher than those in the control group (P < 0.05). 25-oh-vitd3, RI and ovarian PI were positively correlated with HDL-C, and negatively correlated with BMI, LDL-C and fins (P < 0.05). 25-oh-vitd3, RI and ovarian PI were positively correlated with HDL-C, and negatively correlated with glycosylated hemoglobin, LDL-C, CRP, FPG, SBP and TG (P < 0.05). Betatrophin, FSH, LH, 2d-ov, FN and uterus PI were positively correlated with HbA1c, LDL-C, CRP, FPG, SBP and TG, and negatively correlated with HDL-C (P < 0.05). Conclusion: the serum 25-oh-vitd3 level in patients with polycystic ovary syndrome is significantly reduced, betatrophin and FSH are significantly reduced 25-oh-vitd3 was positively correlated with HDL-C and negatively correlated with BMI, LDL-C and fins. Betatrophin, FSH and LH were positively correlated with BMI, LDL-C and fins, and negatively correlated with HDL-C.
