| 摘要 目的 研究输尿管软镜碎石术（FURL）联合钬激光治疗直径≤2.0cm肾结石的效果及对肾功能损伤指标的影响。方法 将我院120例肾结石患者随机均分为FURL组和对照组各60例，分别采用FURL和传统开放手术进行治疗，比较两组结石清除率（SFR）、围术期指标、疼痛程度分级、肾功能指标以及术后并发症。结果 FURL组和对照组SFR分别为95.77%和98.12%（P>0.05），FURL组手术时间、术中出血量、住院时间及住院费用低于对照组，差异有统计学意义（P<0.05）；FURL组术后疼痛程度分级明显低于对照组（P<0.05）；两组术后第1d血清SCr、BMG和RBP水平均明显升高（P<0.05），且FURL组SCr、BMG和RBP低于对照组，差异有统计学意义（P<0.05）；FURL组和对照组并发症发生率分别为8.33%和21.67%（P<0.05）。结论 FURL治疗直径≤2cm肾结石的SFR与传统开放手术相近，同时有利于减轻术后疼痛程度和肾功能损害，减少并发症发生。 |

| Objective To study the effects of flexible ureteroscopic lithotripsy (FURL) combined with holmium laser in the treatment of kidney stones with diameter≤2.0 cm and its influence on the indexes of renal function damage. Methods 120 patients with kidney stones in our hospital were randomly divided into FURL group and control group, with 60 cases in each group. They were treated with FURL and traditional open surgery respectively. The stone free rate (SFR), perioperative indicators, pain level classification, renal function indexes and postoperative complications were compared between the two groups. Results The SFR values in FURL group and control group were 95.77% and 98.12% respectively (P>0.05), and the surgical time, intraoperative blood loss, hospital stay and hospitalization cost in FURL group were lower than those in control group (P<0.05). The classification of postoperative pain level in FURL group was significantly lower than that in control group (P<0.05). The levels of serum SCr, BMG and RBP on the 1st d after surgery in the two groups were significantly increased (P<0.05), and the levels of SCr, BMG and RBP in FURL group were lower than those in control group (P<0.05). The incidence rates of complications in FURL group and control group were 8.33% and 21.67% respectively (P<0.05). Conclusion FURL has similar SFR to traditional open surgery in the treatment of kidney stones with diameter≤2cm, and it is beneficial to relieve the postoperative pain level and renal function damage, and reduce the occurrence of complications. |
