      目的:探讨SMAD7基因rs4939827位点多态性对下肢动脉经皮腔内血管成形术(PTA)+支架植入术(STENT)术后再狭窄的影响。方法:选择2015年7月-2019年6月于本院血管外科接受下肢动脉PTA+STENT的患者200例为研究对象,术后9个月出现再狭窄者90例为观察组,未出现再狭窄者110例为对照组。收集两组患者一般资料,酶联免疫吸附试验(ELISA)法检测两组血清SMAD7表达水平,聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)技术测定SMAD7基因rs4939827位点多态性,分析影响下肢动脉PTA+STENT术后再狭窄的危险因素。结果:与对照组相比,观察组患者术后血清总胆固醇、低密度脂蛋白胆固醇水平、血管管腔直径再狭窄程度、SMAD7基因rs4939827位点C等位基因、CC基因型频率显著升高(P<0.05),血清中SMAD7表达水平、SMAD7基因rs4939827位点T等位基因和TT基因型频率显著降低(P<0.05)。Logistic回归分析显示血清中总胆固醇>4.85mmol/L、低密度脂蛋白胆固醇>2.81mmol/L、血清SMAD7水平<118.34 ng/mL、SMAD7基因rs4939827位点CC基因型为下肢动脉PTA+STENT术后再狭窄的危险因素(P<0.05),SMAD7基因rs4939827位点TT基因型为下肢动脉PTA+STENT术后再狭窄的保护因素(P<0.05)。结论:SMAD7基因rs4939827位点CC基因型和TT基因型与下肢动脉PTA+STENT术后再狭窄有关。

      Objective: To investigate the effects of polymorphism of rs4939827 locus of SMAD7 gene on restenosis after percutaneous transluminal angioplasty (PTA) and stent implantation (STENT) for lower extremity artery. Methods: From July 2015 to June 2019, 200 patients who received PTA + STENT for lower extremity artery in vascular surgery of our hospital were selected as the study objects. 90 patients who had restenosis at 9 months after operation were selected as the observation group, and 110 patients who did not have restenosis were selected as the control group. General data of the two groups were collected, the expression of SMAD7 in serum of the two groups was detected by enzyme-linked immunosorbent assay (ELISA), the polymorphism of rs4939827 locus of SMAD7 gene was determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP), and the risk factors of restenosis after PTA + STENT were analyzed. Results: Compared with those in the control group, the levels of serum total cholesterol and low density lipoprotein cholesterol, restenosis degree of vascular lumen diameter, the frequencies of C allele and CC genotype at rs4939827 locus of SMAD7 gene in the observation group were significantly higher (P < 0.05), and the level of SMAD7 expression in serum, T allele and TT genotype frequency at rs4939827 locus of SMAD7 gene were significantly lower (P < 0.05). Logistic regression analysis showed that the risk factors of restenosis after PTA + STENT for lower extremity artery were total cholesterol > 4.85mmol/L, LDL cholesterol > 2.81mmol/L, serum SMAD7 level < 118.34ng/ml, and CC genotype at rs4939827 of SMAD7 gene (P < 0.05), and TT genotype at rs4939827 of SMAD7 gene was the protective factor of restenosis after PTA + STENT (P < 0.05). Conclusions: The CC genotype and TT genotype of rs4939827 loci on SMAD7 gene are related to restenosis after PTA + STENT for lower extremity artery.
