      目的:探讨N-乙酰半胱氨酸对特发性肺纤维化的临床疗效,并分析其对患者肺泡灌洗液中 TGF-β1、IL-6、VEGF表达水平的影响。方法:收集我院2016年3月-2019年3月70例特发性肺纤维化患者,分析N-乙酰半胱氨酸对患者肺功能、纤维化指标以及肺泡中 TGF-β1、IL-6、VEGF的表达水平;结果:两组FVC、FEV1/FVC、DLCO以及血氧饱和度治疗后两组均有明显改善(P<0.05)。观察组治疗前后PCIII差值为36.18±5.86ng/ml,明显高于对照组的15.64±3.63ng/ml,其余指标CIV、PICP、LN观察组差值均明显高于对照组(P<0.05);TGF-β1、IL-6、VEGF观察组治疗前后TGF-β1、IL-6、VEGF治疗前后的差值显著高于对照组(P<0.05);肺功能指标中,除FEV1外其余指标与TGF-β1均有明显的负相关性(r<-0.5,P<0.05),而纤维化指标中PCIII、CIV、PICP与TGF-β1均有明显的正相关性(r>0.5,P<0.05);IL-6与FEV1、LN无明显相关性;除FEV1及CIV外,其余肺功能与VEGF均表现出明显的负相关性(r<-0.5,P<0.05),与纤维化指标表现性出显著正相关性(r>0.5,P<0.05)。结论:N-乙酰半胱氨酸雾化吸入能够有效改善IPF患者的肺功能,并通过干预TGF-β1、IL-6、VEGF的表达水平改善患者的肺纤维化程度,值得临床推广应用。

      Objective: To analyze the clinical efficacy of N-acetylcysteine ??in patients with idiopathic pulmonary fibrosis, and to analyze its effect on the expression levels of TGF-β1, IL-6 and VEGF in patients with alveolar lavage fluid.Methods: A total of 70 patients with idiopathic pulmonary fibrosis from March 2016 to March 2019 in our hospital were collected and analyzed for N-acetylcysteine ??on lung function, fibrosis indexes, and TGF-β1 and IL-6 in alveoli. VEGF expression level;Results: FVC, FEV1 / FVC, DLCO, and blood oxygen saturation were significantly improved in the two groups after treatment (P <0.05). Pulmonary fibrosis was significantly improved after treatment (P <0.05). The PCIII difference between the observation group before and after treatment was 36.18 ± 5.86ng / ml, which was significantly higher than that of the control group at 15.64 ± 3.63ng / ml. The differences in the PICP and LN observation groups were significantly higher than those in the control group (P <0.05); the differences in the TGF-β1, IL-6, and VEGF observation groups before and after treatment were significantly higher than those in the control group. Group (P <0.05); among the lung function indicators, except for FEV1, all other indicators had a significant negative correlation with TGF-β1 (r <-0.5, P <0.05), while among the fibrosis indicators, PCIII, CIV, PICP and TGF-β1 had a significant positive correlation (r> 0.5, P <0.05); IL-6 had no significant correlation with FEV1 and LN; except for FEV1 and CIV, the other lung functions showed a significant negative correlation with VEGF (R <-0.5, P <0.05), showing a significant positive correlation with fibrosis indicators (r> 0.5, P <0.05).Conclusion: N-acetylcysteine ??aerosol inhalation can effectively improve the lung function of patients with IPF, and improve the degree of pulmonary fibrosis in patients by intervening the expression levels of TGF-β1, IL-6 and VEGF.
