      目的 探究老年糖尿病视网膜病变(DR)患者血清中miR-29b和miR-126水平及相关性研究。方法 选取2017年5月-2019年11月来台州市立医院接受治疗的126例老年DR患者作为研究对象,其中单纯2型糖尿病(DM)组42例,非增殖性糖尿病视网膜病变(NPDR)组40例,增殖性糖尿病视网膜病变(PDR)组44例;另选取40例老年体检者作为对照组。采用实时荧光定量PCR方法检测血清中miR-29b和miR-126水平。比较各组间血清miR-29b、miR-126,血压、血糖及血脂等指标差异,将影响糖尿病患者视网膜病变的指标进行单因素分析,同时应用多因素Logistic逐步回归分析探讨糖尿病视网膜病变的危险因素。结果 四组血清miR-29b和miR-126水平比较差异均有统计学意义(P<0.05),其中PDR组血清miR-29b水平明显高于对照组、DM组及NPDR组,且miR-126水平明显低于对照组、DM组及NPDR组;NPDR组血清miR-29b明显高于DM组和对照组,且miR-126明显低于DM组和对照组(P<0.05);NPDR组及PDR组SBP、DBP明显高于对照组,NPDR组、PDR组及DM组FBG、2hPG、HbA1c、TG、TC、HDL-C水平明显高于对照组,PDR组FBG、2hPG、HbA1c、TG水平明显高于DM组(P<0.05)。单因素分析显示,年龄、血清miR-29b、miR-126、HbA1c、FBG等因素具有统计学意义(P<0.05)。多因素Logistic逐步回归分析,结果显示患者年龄<75岁、血清miR-126<2.43ng/ul为老年糖尿病患者视网膜病变的保护因素,血清miR-29b≥8.38ng/ul、HbA1c≥6.00%、FBG≥8.15mmol/L为老年糖尿病患者视网膜病变的危险因素。结论 老年DR患者血清miR-29b水平升高,而血清miR-126水平降低,二者有望成为老年糖尿病视网膜病变的有效生物标志物。

      Objective To investigate the relationship between serum miR-29b, miR-126 levels and elderly diabetic retinopathy (DR). Methods A total of 126 elderly patients with diabetic retinopathy treated in Taizhou Municipal Hospital from May 2017 to November 2019 were enrolled, including 42 cases of type 2 diabetes mellitus (DM group) and 40 cases of non-proliferative diabetic retinopathy (NPDR group) and 44 cases of proliferative diabetic retinopathy (PDR group). Meanwhile, another 40 elderly cases of health examinees were set as control group. Serum levels of miR-29b and miR-126 were detected by real-time fluorescent quantitative PCR. The differences of serum mir-29b, miR-126, blood pressure, blood glucose and blood lipid were compared among the groups. Single factor analysis was carried out on the indexes of diabetic retinopathy. To explore the risk factors of diabetic retinopathy by stepwise Logistic regression analysis. Results There were significant differences in the levels of mir-29b and miR-126 between the four groups (P<0.05); the level of miR-29b in PDR group was significantly higher than that in control group, DM group and NPDR group, The level of miR-126 was significantly lower than that of control group, DM group and NPDR group; the serum mir-29b in NPDR group was significantly higher than that in DM group and control group, miR-126 was significantly lower in DM group and control group (P<0.05); The SBP and DBP of NPDR group and PDR group were significantly higher than those of the control group, the levels of FBG, 2hPG, HbA1c, TG, TC and HDL-C in NPDR group, PDR group and DM group were significantly higher than those in the control group, The levels of FBG, 2hPG, HbA1c and TG in PDR group were significantly higher than those in DM group (P<0.05). Univariate analysis of the factors affecting the diabetic retinopathy showed that the age, serum miR-29b, miR-126, HbA1c and FBG were statistically significant (P<0.05). Logistic stepwise regression analysis showed that patients with age <75 year-old and serum miR-126<2.43ng/ul were protective factors for elderly diabetic retinopathy. Serum miR-29b≥8.38ng/ul, HbA1c≥6.00%, and FBG≥8.15mmol/L were risk factors for elderly patients with diabetic retinopathy. Conclusion Elevated serum miR-29b levels and decreased serum miR-126 levels are related to the development and progression of DR patients, and both are expected to be effective biomarkers of diabetic retinopathy in the elderly.
