      摘要: 目的 调查老年女性2型糖尿病(T2DM)患者的血清骨转换标志物、骨密度和未来10年骨质疏松性骨折风险。方法 对306例老年(≥60岁)女性的一般情况进行问卷调查,并进行血清性激素、血生化、骨转换标志物和骨密度检测,根据骨折风险预测工具计算每位老年女性未来10年主要骨质疏松性骨折风险(PMOP)和髋部骨折风险(PHF),然后根据是否患有T2DM进行分组和统计分析。结果 与非T2DM组相比,T2DM组血清I型前胶原氨基端前肽、I型胶原羧基末端肽交联、骨钙素、总胆固醇、低密度脂蛋白胆固醇和血钠水平较低,股骨颈骨密度、总髋部骨密度、雌二醇和血糖水平较高,差异均有统计学意义(均P<0.05),但在未来10年PMOP和PHF上差异无统计学意义(P >0.05)。 结论 T2DM可能有助于延缓老年女性骨质的丢失,不会增加其骨质疏松性骨折的风险。

      Objective To explore bone turnover markers, bone mineral density and ten years probability of osteoporotic fracture in elderly women with type 2 diabetes mellitus (T2DM). Methods A total of 306 elderly women were subject to surveys on underlying questionnaire survey, and determination of sex steroids, biochemical parameters, bone turnover markers, and bone mineral density. The fracture risk assessment tool was applied to calculate ten years probability of major osteoporotic fracture (PMOF) and probability of hip fracture (PHF). The difference on above observation between T2DM group and non-T2DM group were analysed. Results The level of serum N-terminal propeptide of type 1 procolagen, C-terminal telopeptide of type 1 collagen, osteocallin, total cholesterol, low-density lipoprotein cholesterol, and sodium were significantly lower, while the level of femoral neck bone mineral density, total hip bone mineral density, estradiol, and serum glucose were significantly higher in T2DM group than non-T2DM group(all P<0.05). In addition, there was no significant difference in PMOF and PHF(all P >0.05). Conclusions T2DM could slow down bone loss, therefore not increasing the probability of osteoporotic fracture in elderly women with T2DM.
