      目的 探讨自分泌运动因子-溶血磷脂酸(ATX-LPA)和丝裂原活化蛋白激酶(MAPK) 信号通路与类风湿性关节炎(RA)临床征象的关系。方法 以RA患者72例为RA组,同期30例膝骨关节炎患者(OA)为OA组和30例健康查体志愿者为健康组。ELISA法检测三组血清ATX、LPA、MAPK、类风湿因子(RF)表达水平。分析RA组疾病活动度、疼痛程度等临床征象。比较RA组病情高度活动患者(DAS28评分>5.1分)和病情低中度活动(DAS28评分≤5.1分)患者血清ATX、LPA、MAPK、RF水平以及VAS评分,并分析RA组患者血清ATX、LPA、MAPK水平与血清RF水平、DAS28评分以及VAS评分的关系。结果 与健康组比较,RA组血清ATX、LPA、MAPK、RF水平均升高(P<0.05)。与OA组比较,RA组血清ATX、LPA、RF水平均升高(P<0.05)。RA组和OA组血清MAPK水平无明显差异,OA组与健康组血清ATX、LPA、RF水平亦无明显差异(P>0.05)。OA组血清MAPK水平高于健康组(P<0.05)。与RA组病情低中度活动患者比较,其病情高度活动患者血清ATX、LPA、MAPK、RF水平以及VAS评分均升高(P<0.05)。Pearson线性相关分析结果显示,RA组血清ATX、LPA、MAPK水平与血清RF水平、DAS28评分以及VAS评分均呈正相关(P<0.05)。结论 ATX-LPA和MAPK 信号通路与RA疾病活动度、疼痛程度等临床征象以及血清病情评估指标均密切相关,可能为RA防治提供新靶点。

      Objective: To investigate the relationship between autocrine kinetics-lysophosphatidic acid (ATX-LPA) and mitogen-activated protein kinase (MAPK) signaling pathways with clinical signs of rheumatoid arthritis (RA). Methods: 72 patients with RA were selected as RA group, 30 patients with knee osteoarthritis (OA) were selected as OA group and 30 healthy volunteers were selected as health group. The expression levels of ATX, LPA, MAPK and rheumatoid factor (RF) of 3 groups were detected by ELISA. The clinical symptoms as disease activity and pain in RA group were analyzed. The serum ATX, LPA, MAPK, RF levels and VAS scores of patients with high disease activity (DAS28 score > 5.1 score) and patients with low and moderate disease activity (DAS28 score < 5.1 score) were compared. The relationship between serum ATX, LPA, MAPK levels with serum RF, DAS28 score and VAS score was analyzed. Results: Compared with those of the healthy group, serum ATX, LPA, MAPK and RF levels in RA group were increased (P < 0.05). Compared with those of the OA group, serum ATX, LPA and RF levels in RA group increased (P < 0.05). There was no significant difference in serum MAPK levels between RA group and OA group; And there was no significant difference in serum ATX, LPA and RF levels between OA group and healthy group (P > 0.05). The serum level of MAPK in OA group was higher than that in healthy group (P < 0.05). Compared with those of the patient with low and moderate disease activity in RA group, the serum levels of ATX, LPA, MAPK and RF and VAS score in patients with high disease activity increased (P < 0.05). Pearson linear correlation analysis showed that serum ATX, LPA and MAPK levels were positively correlated with serum RF level, DAS28 score and VAS score in RA group (P < 0.05). Conclusion: ATX-LPA and MAPK signaling pathways are closely related to clinical symptoms such as disease activity, pain degree and serum disease assessment indicators of RA, which may provide a new target for RA prevent and treatment.
