      目的 探讨使用两种不同浓度的罗哌卡因进行椎管内麻醉后,实施外倒转术的临床效果研究。方法 选取48例足月单胎臀位的孕妇为研究对象,随机分为2组,对照组和观察组,各24例。对照组应用10mg/ml的罗哌卡因15mg 1:1稀释后进行椎管内麻醉,麻醉成功后行外倒转术;观察组应用10mg/ml的罗哌卡因10mg 1:2稀释后进行椎管内麻醉,麻醉成功后行外倒转术。比较两组的外倒转成功率、紧急剖宫产率、病人的疼痛评分、低血压发生情况。结果 观察组外倒转成功17例,对照组外倒转成功19例,差异无统计学意义。观察组紧急剖宫产3例,对照组紧急剖宫产2例,差异无统计学意义。观察组病人的疼痛评分2.00±0.98,对照组病人的疼痛评分1.79±1.10,差异无统计学意义。观察组出现低血压5例,对照组出现低血压16例,差异具有统计学意义。结论 椎管内麻醉使用小剂量罗哌卡因,能明显降低外倒转术中病人的麻醉药副反应,但并未降低手术成功率,也未增加病人的疼痛不适,值得推广。

      Objective To investigate the clinical effect of using two different concentrations of ropivacaine in spinal anesthesia in the procedure of external cephalic inversion. Methods 48 full-term singleton pregnant women with breech presentation were included. They were divided into two groups, the control group and the observation group, with 24 cases each. In the control group, external cephalic inversion was performed after successful spinal anesthesia with 15 mg of ropivacaine diluted 1: 1; In the observation group, 10 mg of ropivacaine was diluted 1: 2 for spinal anesthesia. The success rate of external cephalic inversion, emergency cesarean section rate, patient's pain score, and incidence of hypotension were compared between the two groups. Results In the observation group there were 17 cases of successful inversion and 3 cases of emergency cesarean section. While in the control group, there were 19 cases and 2 cases respectively. The pain score of patients in the observation group was 2.00 ± 0.98, which was 1.79 ± 1.10 in the control group. Compared the success rate of external inversion, emergency cesarean section rate and patient's pain score between the two groups, there were no significant differences. As for hypotension, there were 5 cases in the observation group compared with 16 cases in the control group. The difference was statistically significant. Conclusion Using low-dose ropivacaine in spinal anesthesia could significantly reduce the side effects of anesthesia in patients undergoing external cephalic inversion, but it did not reduce the success rate of procedure and did not increase patients’ pain and discomfort. Low-dose ropivacaine in spinal anesthesia worth being applied in the procedure of external cephalic inversion.
