      目的 观察加味黄风汤对糖尿病肾病大鼠肾脏8-羟基脱氧鸟苷(8-OHdG)表达水平的影响。方法 雄性SD大鼠经腹腔注射链脲佐菌素(STZ)造模,随机分为正常对照组、模型组、加味黄风汤低剂量组、高剂量组各10只,8周后检测24小时尿微量白蛋白,检测血清肌酐及血糖水平。肾组织病理切片光镜及电镜检查。酶联免疫吸附测定法(ELISA)检测大鼠肾组织中8-OHdG的表达水平,化学发光法(Luminescence)检测大鼠肾组织中腺嘌呤核苷三磷酸(ATP)水平。结果:模型组大鼠24小时微量白蛋白尿较对照组增加[(1702.06±659.75)μg比(503.03±320.16)μg,P<0.05],肾脏病理可见肾小球肥大,系膜基质增生,基底膜增厚及足细胞足突融合,模型组大鼠肾组织8-OhdG表达水平较正常对照组增加[(795.23±185.73)ng/mL比(600.79±159.54)ng/mL,P<0.05)],肾组织ATP水平较对照组下降[(0.27±0.17)pmol/mg比(0.72±0.14)pmol/mg,P<0.05)]。加味黄风汤治疗8周后糖尿病肾病大鼠24小时尿微量白蛋白水平明显下降[(783.34±326.11)μg比(1567.46±621.97)μg,(610.99±325.54)比(1610.34±695.64)μg,P<0.05μg];血糖水平明显下降[(24.31±1.90)mmol/L比(29.19±1.88)mmol/L,(23.97±2.73)mmol/L比(30.56±1.88)mmol/L,P<0.05];光镜下肾小球肥大、系膜基质增生减轻;电镜下足细胞足突融合、基底膜增厚情况都有所改善。低剂量及高剂量治疗组较模型组8-OhdG表达水平均有所下降,有统计学意义[(695.49±159.76)ng/mL比(795.23±185.73)ng/mL,(610.61±71.43)ng/mL比(795.23±185.73)ng/mL,P<0.05]。高剂量组8-OhdG表达水平低于低剂量组,但二者无统计学意义[(610.61±71.43)ng/mL比(695.49±159.76)ng/mL,P>0.05]。治疗组较模型组ATP表达水平均有所上升,有统计学意义[(0.39±0.15)pmol/mg比(0.27±0.17)pmol/mg,(0.52±0.14)pmol/mg比(0.27±0.17)pmol/mg,P<0.05]。高剂量组ATP表达水平高于低剂量组,但二者无统计学意义[(0.52±0.14)pmol/mg比(0.39±0.15)pmol/mg,P>0.05]。结论 糖尿病肾病大鼠肾组织8-OHdG表达明显上升,ATP表达水平下降,出现蛋白尿,足细胞损伤。加味黄风汤可下调糖尿病肾病大鼠肾组织的8-OHdG表达水平,提高肾组织的ATP水平,减轻蛋白尿水平,并可改善血糖,保护肾功能。

      Objective: To observe the effect of Jiawei Huangfeng Decoction on the expression of 8-OHdG in kidney tissue of diabetic nephropathy rats. Methods: SD rats were injected intraperitoneally streptozotocin (STZ) to make diabetic nephropathy model, divided into normal control group, model group, Jiawei Huangfeng Decoction low dose group and high dose group of 10 rats each. After 8 weeks, the 24-hour urine microalbumin, serum creatinine and blood glucose levels,light microscope and electron microscope of the renal pathological changes were oberved. ELISA was used to detect the expression level of 8-hydroxydeoxyguanosine (8-OHdG) in rat kidney. Luminescence was used to detect ATP levels in rat kidney tissue.ResμLts: 24-hour Urinary microalbuminuria was increased in the model group[(1702.06±659.75)μg vs (503.03±320.16)μg,P<0.05]. GlomerμLar hypertrophy, mesangial matrix hyperplasia, basement membrane thickening and podocyte foot process fusion were observed in renal pathology. The expression level of 8-OhdG in the renal tissue of the model group was significantly higher than that of the normal control group[(795.23±185.73)ng/mL vs (600.79±159.54)ng/mL,P<0.05)]. The expression level of ATP in the renal tissue of the model group was significantly higher than that of the normal control group[(0.27±0.17)pmol/mg vs (0.72±0.14)pmol/mg,P<0.05)]. After treatment with Jiawei Huangfeng Decoction for 8 weeks, 24-hour Urinary microalbuminuria was significantly decreased[(783.34±326.11)μg vs (1567.46±621.97)μg,(610.99±325.54)μg vs (1610.34±695.64)μg ,P<0.05]. The blood glucose level decreased significantly, which was statistically significant [(24.31±1.90)mmol/L vs (29.19±1.88)mmol/L,(23.97±2.73)mmol/L vs (30.56±1.88)mmol/L,P<0.05]. There was no significant difference in serum creatinine between the groups (P>0.05).GlomerμLar hypertrophy and mesangial matrix hyperplasia were alleviated under light microscope. Electron microscope observed podocyte foot process fusion, basement membrane thickening had been improved. The expression levels of 8-OhdG in the low-dose and high-dose groups were significantly lower than those in the model group [(695.49±159.76)ng/mL vs (795.23±185.73)ng/mL,(610.61±71.43)ng/mL vs (795.23±185.73)ng/mL,P<0.05]. The expression level of 8-OhdG in the high-dose group was lower than that in the low-dose group, but the two were not statistically significant [(610.61±71.43)ng/mL vs(695.49±159.76)ng/mL,P>0.05].The expression levels of ATP in the low-dose and high-dose groups were significantly higher than those in the model group[(0.39±0.15)pmol/mg vs (0.27±0.17)pmol/mg,(0.52±0.14)pmol/mg vs (0.27±0.17)pmol/mg,P<0.05]. The expression level of ATP in the high dose group was higher than that in the low dose group, but the two were not statistically significant [(0.52±0.14)pmol/mg vs (0.39±0.15)pmol/mg,P>0.05]. Conclusion: Modified Huangfeng decoction can down-regμLate the expression level of 8-OHdG in renal tissue of rats with diabetic nephropathy, increase the level of ATP in renal tissue, reduce the level of proteinuria, and improve blood sugar,and protect kidney function.
