      目的:研究血清中结缔组织生长因子(CTGF)和血小板衍生生长因子-BB(PDGF-BB)表达与老年特发性间质性肺炎(IIP)的相关性。方法:选取我院呼吸内科2015年3月至2019年10月期间收治的99例老年IIP患者作为研究对象,根据不同病理类型分为特发性肺纤维化53例(IPF组)和除IPF外的其他类型IIP 46例(nIPF-IIP组)两个亚组,其中nIPF-IIP组包括非特异性间质性肺炎(NSIP) 18例,隐源性机化性肺炎(COP) 17例,呼吸性细支气管炎伴间质性肺疾病(RB-ILD) 11例;另外选取40例同时间段在我院进行老年健康体检者作为对照组。比较所有患者临床资料,并选择酶联免疫吸附试验法(ELISA)分别测定血清CTGF和PDGF-BB表达指标,同时分析血清CTGF和PDGF-BB表达与特发性间质性肺炎的ROC曲线分析的诊断界限值,并采用logistic回归分析探讨老年特发性间质性肺炎肺纤维化相关影响因素。结果:IPF组血清hs-CRP、Hcy、CTGF和PDGF-BB表达水平明显高于对照组和nIPF-IIP组,且nIPF-IIP组血清hs-CRP、Hcy、CTGF和PDGF-BB表达水平明显高于对照组,各组比较差异均有统计学意义(P<0.05);根据ROC曲线显示:血清CTGF表达对于鉴别IPF组和nIPF-IIP组以及对照组和nIPF-IIP组的敏感度和特异度分别为(82.37%、85.54%,84.25%、85.31%),而血清PDGF-BB表达鉴别IPF组和nIPF-IIP组以及nIPF-IIP组和对照组的敏感度和特异度分别为(80.56%、87.73%,81.27%、86.52%);Logistic回归分析结果显示,血清hs-CRP、Hcy、CTGF、PDGF-BB升高均为老年IIP患者肺纤维化的危险因素(P<0.05)。结论:血清CTGF和PDGF-BB在老年特异性间质性肺炎患者中表达升高,二者联合检测能提高诊断的准确性,同时二者表达升高可以作为预测老年特异性间质性肺纤维化发生风险的标志物。

      Objective To investigate the correlation between serum connective tissue growth factor (CTGF) and platelet-derived growth factor-BB (PDGF-BB) expression and idiopathic interstitial pneumonia (IIP) in elder patients. Methods 99 IIP elder patients treated in the Respiratory Department of our hospital from March 2015 to October 2019 were selected and divided into two subgroups: IPF group (n=53) and IIP group (n=46) according to different pathological types. The nIPF-IIP group included 18 cases of nonspecific interstitial pneumonia(NSIP), 17 cases of Cryptogenic Organizing Pneumoia(COP), and 11 cases of RB-associated ILD(RB-ILD), meanwhile, another 21 case of heathy examinees in our hospital were selected as control group. The clinical data of all patients were compared, and the expression of serum CTGF and PDGF-BB were measured by ELISA. The diagnostic ROC threshold of serum CTGF and PDGF-BB expression was analyzed. Then the multivariate logistic stepwise regression analysis was used to determine the related factors of pulmonary fibrosis in elderly patients with idiopathic interstitial pneumonia. Results The levels of hs-CRP, Hcy, CTGF and PDGF-BB in serum of IPF group were significantly higher than those of control group and nIPF-IIP group, and the levels of hs-CRP, Hcy, CTGF and PDGF-BB in serum of nIPF-IIP group were significantly higher than those of control group with significant differences among each group (P<0.05). According to the ROC curve, the specificity and sensitivity of serum CTGF expression in the identification between IPF group and nIPF-IIP group as well as nIPF-IIP group and control group were (82.37%, 85.54%; 84.25%, 85.31%), respectively, which were (80.56%, 87.73%; 81.27%, 86.52%) for PDGF-BB expression. Logistic regression analysis showed that high level of serum hs-CRP, Hcy, CTGF and PDGF-BB were risk factors for post-pulmonary fibrosis in IIP patients(P<0.05). Conclusion The expression of CTGF and PDGF-BB in serum are high in elderly patients with specific interstitial pneumonia. The combined detection of CTGF and PDGF-BB can improve the accuracy of diagnosis. At the same time, the increased expression of CTGF and PDGF-BB can be used as a marker to predict the risk of elderly specific interstitial pulmonary fibrosis.
