      【摘要】 目的:探讨肥胖儿童血尿酸水平,以及其与胰岛素抵抗、腰围等肥胖相关因素的相关性。方法:选取2016年3月-2019年3月在我院就诊,临床诊断为肥胖症的5-16岁汉族儿童,作为肥胖组,共85例。另招募128例性别和年龄匹配的健康儿童作为对照组。比较2组间血尿酸水平和高尿酸血症(HUA)患病率。探讨尿酸与胰岛素抵抗指数(HOMA-IR)、人体形态学测量参数指标和血检指标的相关性。结果:肥胖组HUA患病率明显高于对照组(28 % vs.2.4 %,P<0.05)。Pearson 相关分析提示UA与体重指数、体重、腰围、胰岛素抵抗、空腹胰岛素、糖化血红蛋白、空腹C肽、有黑棘皮和脂肪肝呈正相关,与高密度脂蛋白胆固醇(HDL-C) 呈负相关(P<0.05)。Logistic二元回归分析提示体重、空腹血糖、脂肪肝是高尿酸血症的危险因素。结论:肥胖患儿明显表现出HUA同时伴有IR,并且血UA水平随着体重指数的增加而升高,进而IR进一步加重。

      Abstract Objective: To investigate the serum uric acid level in obese children and its correlation with obesity-related factors such as insulin resistance and waist circumference. Methods: A total of 85 Han children aged 5-16 years who were diagnosed as obese in our hospital from March 2016 to March 2019 were selected. Another 128 healthy and age-matched healthy children were recruited as controls. The serum uric acid level and the prevalence of hyperuricemia (HUA) were compared between the two groups. To explore the correlation between serum uric acid and insulin resistance index (HOMA-IR), human morphological parameters and blood tests. Results: The prevalence of HUA was significantly higher in the obese group than in the control group (28% vs. 2.4%, P <0.05). Pearson correlation analysis showed that SUA was positively correlated with body mass index, body weight, waist circumference, insulin resistance, fasting insulin, glycated hemoglobin, fasting C-peptide, black echinoderm and fatty liver, and negatively correlated with high-density lipoprotein cholesterol (HDL-C) (P <0.05). Logistic binary regression analysis suggested that weight, fasting blood glucose, and fatty liver were risk factors for hyperuricemia. Conclusion: Obese children clearly show that HUA is accompanied by IR, and blood UA levels increase with increasing body mass index, and further increase IR.
