| 摘 要：[目的] 评价优化经皮椎弓根螺钉内固定技术操作步骤在胸腰椎工作内固定术中的临床疗效。[方法] 2016年10月～2019年1月收治的胸腰椎骨折患者共52例,根据时间分为两组。常规组患者应用7步透视法置入椎弓根螺钉，优化组患者应用3步透视法置入椎弓根螺钉内固定治疗。比较两组临床资料。［结果］两组患者均顺利完成手术，术中均未出现医源性神根损伤等严重并发症。优化组手术时间、透视次数显著低于常规组，术中出血、术后伤椎前缘高度比值、术后椎体高度丢失率两组间均无显著差别。两组患者术前术后VAS评分均有显著差别。［结论］优化经皮椎弓根螺钉内固定术操作步骤能有效缩短手术时间，减少术中透视次数，并能保持常规透视组相同的治疗疗效，值得推广。 |

| ABSTRICT [Objective] To evaluate the clinical effect of optimizing the operation steps of percutaneous pedicle screw internal fixation technique in Thoracolumbar fracture internal fixation. [Methods] A total of 52 patients with thoracolumbar fractures treated from October 2016 to January 2019 were divided into two groups according to time. Patients in the conventional group received pedicle screws with 7-step fluoroscopy, and patients with anterior and posterior groups received pedicle screws with 3-step fluoroscopy. Compare clinical data between the two groups. [Result] Surgery was successfully completed in both groups, and no serious complications such as iatrogenic sacral injury occurred during the operation. The operation time and the number of fluoroscopy in the anterior and posterior groups were significantly lower than those in the conventional group. There were no significant differences between the two groups in terms of intraoperative bleeding, postoperative vertebral leading edge height ratio, and postoperative vertebral height loss rate. There were significant differences in VAS scores before and after surgery in both groups. [Conclusion] Optimizing the procedure of percutaneous pedicle screw internal fixation can effectively shorten the operation time, reduce the number of intraoperative fluoroscopy, and maintain the same therapeutic effect in the conventional fluoroscopy group, which is worthy of promotion. |
