| 目的: 探讨结核RNA检测(TB SAT)在肺结核早期诊断中的应用价值?方法: 2016年1月-2017年6月永康市第一人民医院收治的疑似结核病患者272例，对痰或支气管肺泡灌洗液标本进行涂片、培养和TB SAT检测?结果: 272例患者中，培阳肺结核患者85例，培阴经过临床诊断肺结核患者95例，非结核患者92例。以临床诊断作为金标准，涂片、培养、TB SAT检测诊断肺结核的敏感度分别为26.11%、47.22% 及59.44%；TB SAT的检测灵敏度明显高于涂片（χ2=40.852,P<0.001）和培养（χ2=5.402,P<0.05）；TB SAT诊断涂阴肺结核的敏感性为45.11%，特异性为98.91%，阳性预测值为98.36%，阴性预测值为55.49%。TB SAT检测阳性患者治疗3个月、6个月后，TB SAT阳性率阳性率明显下降（P均<0.05）。结论: TB SAT检测敏感度较高、特异度好，可能有助于提升肺结核患者的早期检出率，具有较好的临床应用价值。 |

| Objective: To explore the value of RNA detection(TB SAT) in the early diagnosis of pulmonary tuberculosis. Methods: 272 cases of suspected tuberculosis patients were recrutied in the first people's Hospital of Yongkang from January 2016 to June -2017.Sputum specimens or bronchoalveolar lavage fluid each patient provided were performed smear，Bactec MGIT 960 culture and TB SAT assay.Results:Of 272 PTB suspects included in the final analysis，85 were TB cuIture positive，95 were PTB patients by clinical diagnosis with culture negative，92 were non-TB patients.Using clinical diagnosis result as the gold standard，the sensitivities of smear，culture and TB SAT were 26.11％，47.22％ and 59.44，respectively．The sensitivity of TB SAT was higher than smear（χ2=40.852,P<0.001）and culture（χ2=5.402,P<0.05）．The sensitivity,specificity，positive predictive value，negative predictive value of TB SAT for smear-negative PTB were 45.11%、98.91%、98.36% and 55.49%.The positive rates of TB SAT were significantly decreased after 3 months and 6 months therapy(P all <0.05).Conclusion: The detection of RNA has high sensitivity and specificity for the early diagnosis of tuberculosis and may help improve the positive rate,which has a good clinical application value. |
