| 目的：探讨益髓醒神针刺联合镜像视觉反馈（MVF）治疗脑卒中后单侧空间忽略（USN）的康复情况。方法：以2016年6月-2018年12月杭州市中医院康复科脑卒中后左脑偏瘫的患者为研究对象，采用随机数字表法分为两组。对照组（38例）进行镜像视觉反馈（MVF）训练联合常规治疗与康复训练，干预组（39例）在对照组基础上加用益髓醒神针刺治疗，均治疗4周。通过对比两组治疗前及4周后SUN程度、肢体运动功能、日常生活能力、认知功能，焦虑、抑郁情绪，以及生活质量评估患者康复效果。结果：两组一般资料差异均无统计学意义（均P＞0.05）。经过4周治疗后，与治疗前相比，两组患者USN程度均出现明显下降（P<0.01）；肢体运动功能、日常生活能力和精神认知功能均明显提高（P<0.01）；焦虑、抑郁情绪得到缓解（P<0.01）；生活质量得到提高（P<0.05）。并且两组比较，研究组在SUN程度、肢体运动功能、日常生活能力、认知功能，负性情绪各方面均进一步得到改善（P<0.05）；生活质量方面在生理机能、生理职能、精力、社会功能及一般健康维度评分上提高更加明显（P<0.05）。结论：益髓醒神针联合MVF对卒中后USN患者具有协同康复作用，可提高患者USN、肢体运动功能、日常生活能力、精神认知，改善患者负性情绪，提高生活质量，值得临床进一步推广。 |

| Objective: To investigate the rehabilitation of unilateral spatial neglect (USN) after stroke by combination of yishui xingshen acupuncture and mirror visual feedback (MVF). Methods: patients with left cerebral hemiplegia after cerebral apoplexy in the rehabilitation department of hangzhou hospital of traditional Chinese medicine from June 2016 to December 2018 were selected as research objects and divided into two groups by random number table method. The control group (38 cases) received MVF training combined with conventional treatment and rehabilitation training, and the intervention group (39 cases) received Yisuixingshen acupuncture therapy based on the control group, for a total of 4 weeks. In addition, effect of rehabilitation was evaluated by comparing the degree of SUN, motor function, daily living ability, cognitive function, anxiety, depression, and quality of life of the two groups before and 4 weeks after treatment. Results: There was no statistically significant difference in general data between the two groups (P>0.05). After 4 weeks of treatment, the USN degree in both groups was significantly decreased compared with that before treatment (P<0.01). Limb motor function, daily living ability and mental cognitive function were significantly improved (P<0.01). Anxiety and depression were relieved (P<0.01), and quality of life was improved (P<0.05). Compared with the two groups, the SUN degree, limb motor function, daily living ability, cognitive function and negative emotion of the research group were further improved (P<0.05). In terms of quality of life, the scores of physiological function, physiological function, energy, social function and general health dimensions were significantly improved (P<0.05). Conclusion: Yisuixingshen acupuncture combined with MVF has a synergistic effect on rehabilitation of USN patients after stroke. which can improve the rehabilitation effect of USN, limb motor function, daily living ability and mental cognition of patients, improve the negative emotions of patients and quality of life, which is worthy of further clinical promotion. |
