      目的:探讨环氧化酶-2(COX-2)和Ki-67在老年甲状腺癌组织中表达及临床意义。方法:选择我院于2017年6月至2019年6月期间行手术治疗的老年甲状腺癌组织87例作为实验组;选择我院于2017年6月至2019年5月期间行手术治疗甲状腺良性病变的正常甲状腺组织83例作为对照组。采用免疫组织化法测定COX-2和Ki-67表达阳性率;采用Western Bolt测定COX-2和Ki67蛋白表达灰度值。比较两组COX-2和Ki-67表达阳性率、COX-2和Ki67蛋白表达灰度值,不同病理特征COX-2和Ki-67表达阳性率、COX-2和Ki67蛋白表达灰度值。结果:实验组COX-2和Ki-67表达阳性率高于对照组(P<0.05)。实验组COX-2和Ki67蛋白表达灰度值高于对照组(P<0.05)。不同性别、年龄和TNM分期COX-2和Ki-67表达阳性率比较差异无统计学意义(P>0.05);有包膜浸润和淋巴结转移COX-2和Ki-67表达阳性率高于无包膜浸润和淋巴结转(P<0.05)。不同性别、年龄和TNM分期COX-2和Ki-67蛋白表达灰度值比较差异无统计学意义(P>0.05);有包膜浸润和淋巴结转移蛋白表达灰度值高于无包膜浸润和淋巴结转(P<0.05)。结论:老年甲状腺癌组织中COX-2和Ki-67表达升高,COX-2和Ki-67异常表达参与甲状腺的发生和发展过程,且与淋巴转移和包膜浸润相关,认为其可作为判断甲状腺癌预后参考指标。

      Objective: To investigate the expression and clinical significance of cyclooxygenase-2 (COX-2) and Ki-67 in elderly thyroid cancer. Methods: The 87 elderly patients with thyroid cancer who underwent surgical treatment in our hospital from June 2017 to June 2019 were selected as the experimental group, and 83 normal thyroid tissues who underwent surgical treatment in our hospital from June 2017 to May 2019 were selected as the control group. The positive rates of COX-2 and Ki-67 expression were determined by immunohistochemical method, and the gray values of COX-2 and Ki67 expression were measured by Western Bolt. The positive rates of COX-2 and Ki-67 expression, the gray values of COX-2 and Ki67 expression, the positive rates of COX-2 and Ki-67 expression and the gray values of COX-2 and Ki67 expression in different pathological characteristics were compared between the two groups. Results: The positive rates of COX-2 and Ki-67 expression in the experimental group were higher than those in the control group (P<0.05). The expression of COX-2 and Ki67 protein in experimental group was higher than that in control group (P<0.05). There was no significant difference in the positive rates of COX-2 and Ki-67 in different gender, age and TNM stages (P>0.05); the positive rates of COX-2 and Ki-67 in patients with capsular infiltration and lymph node metastasis were higher than those in patients without capsular infiltration and lymph node metastasis (P<0.05). There was no significant difference in the expression of COX-2 and Ki-67 in different gender, age and TNM stages (P>0.05). The expression of COX-2 and Ki-67 in lymph node metastasis and capsular infiltration was higher than that in non-capsular infiltration and lymph node metastasis (P<0.05). Conclusion: the expression of COX-2 and Ki-67 is increased in the elderly thyroid cancer tissues. The abnormal expression of COX-2 and Ki-67 is involved in the occurrence and development of thyroid, and is related to lymph node metastasis and capsule infiltration. It is considered that COX-2 and Ki-67 can be used as a reference index to judge the prognosis of thyroid cancer.
