      目的 研究督脉电针联合康复训练对脊髓损伤大鼠神经营养因子如神经生长因子(NGF)、碱性成纤维细胞生长因子(bFGF)、脑源性神经营养因子(BDNF)表达的影响,观察两种方法的协同效应。方法 将64只成年健康大鼠随机平均分成对照组、督脉电针组、康复训练组、督脉电针联合康复训练组,每组再分成4周组、8周组两个亚组,共8组,每组8只。建立大鼠脊髓损伤模型,各治疗组给予不同方案干预,对各组大鼠分别采用Basso,Beattie and Bresnahan行为评分法(BBB)评分,采用蛋白免疫印迹法(WB)测定NGF、bFGF、BDNF的表达。结果BBB评分:4周亚组中,督脉电针组、督脉电针联合康复训练组BBB评分较对照组明显升高[(13.26±0.38)、(15.35±0.42)比(6.48±0.37),P均<0.05],康复训练组与对照组比较,差异无统计学意义(P>0.05)。8周亚组中,督脉电针组、康复训练组、督脉电针联合康复训练组BBB评分较对照组明显升高[(16.41±0.75)、(13.53±1.20)、(18.94±0.92)比(9.21±0.51),P均<0.05],且督脉电针联合康复训练组高于督脉电针组、康复训练组[(18.94±0.92)比(16.41±0.75)、(13.53±1.20),P均<0.05];与督脉电针联合康复训练4周组相比,督脉电针联合康复训练8周组BBB评分升高更显著[(18.94±0.92)比(15.35±0.42),P<0.05]。NGF、bFGF、BDNF的表达:4周亚组中,督脉电针组、督脉电针联合康复训练组显著高于对照组 [NGF:(0.45±0.04)、(0.60±0.01)比(0.26±0.05);bFGF:(0.52±0.04)、(0.59±0.02)比(0.31±0.03);BDNF:(0.47±0.07)、(0.61±0.07)比(0.23±0.04);P均<0.05];康复训练组与对照组比较,差异无统计学意义(P>0.05)。8周亚组中,督脉电针组、康复训练组、督脉电针联合康复训练组表达明显高于对照组 [NGF:(0.67±0.09)、(0.59±0.02)、(0.85±0.10)比(0.31±0.01);bFGF:(0.64±0.05)、(0.53±0.02)、(0.77±0.06)比(0.30±0.05);BDNF:(0.66±0.01)、(0.58±0.05)、(0.75±0.05)比(0.24±0.06);P均<0.05],且督脉电针联合康复训练组高于督脉电针组、康复训练组[NGF:(0.85±0.10)比(0.67±0.09)、(0.59±0.02);bFGF:(0.77±0.06)比(0.64±0.05)、(0.53±0.02);BDNF:(0.75±0.05)比(0.66±0.01)、(0.58±0.05);P均<0.05]。结论 对脊髓损伤大鼠,督脉电针、康复训练通过增加受损脊髓神经营养因子的表达促进其功能恢复,且督脉电针及康复训练二者联合存在协同性。

      Objective To study the effect of Du meridian electroacupuncture combined with rehabilitation training on the expression of neurotrophic factors such as nerve growth factor (NGF), basic fibroblast growth factor (bFGF) and brain derived neurotrophic factor(BDNF) in rats with spinal cord injury, and to observe the synergistic effect of the two. Methods A total of 64 healthy adult rats were randomly divided into four groups, namely, the control group, the Du meridian electroacupuncture group, the rehabilitation training group,and the Du meridian electroacupuncture combined rehabilitation training group. Each group was further divided into two subgroups, namely the 4-week group and the 8-week group, with 8 rats in each group. . A model of spinal cord injury was established in rats, and different intervention programs were given to each treatment group. Basso,Beattie and Bresnahan Locomotor Rating scale(BBB) were used to score the rats in each group, and the expressions of NGF, bFGF and BDNF were determined by Western Blot(WB). Results BBB score: in the 4-week subgroup, BBB score in the Du meridian electroacupuncture group and Du meridian electroacupuncture combined rehabilitation training group was significantly higher than that in the control group [(13.26±0.38), (15.35±0.42) ratio (6.48±0.37), all P <0.05], there was no significant difference between the rehabilitation training group and the control group (P>.05). In the 8-week subgroup, BBB scores in the Du meridian electroacupuncture group, rehabilitation training group and Du meridian electroacupuncture combined with rehabilitation training group were significantly higher than those in the control group [(16.41±0.75), (13.53±1.20), (18.94±0.92) ratio (9.21±0.51), all P <0.05]. In addition, the combined rehabilitation training group was higher than the two groups [(18.94±0.92) compared with (16.41±0.75) and (13.53±1.20), all P <0.05].compared with the 4-week group, the BBB score of the 8-week group increased significantly [(18.94±0.92) vs. (15.35±0.42), P<0.05]. Expression of NGF, bFGF and BDNF: In the 4-week subgroup, the ratio of the two groups was significantly higher than that of the control group [NGF:(0.45±0.04), (0.60±0.01), (0.26±0.05); bFGF :(0.52±0.04), (0.59±0.02), (0.31±0.03); BDNF :(0.47±0.07), (0.61±0.07), (0.23±0.04); all P <0.05]. There was no significant difference between the rehabilitation training group and the control group (P>0.05). In the 8-week subgroup, the expression of the Du meridian electroacupuncture group, rehabilitation training group and Du meridian electroacupuncture combined with rehabilitation training group was significantly higher than that of the control group [NGF:(0.67±0.09) ,(0.59±0.02) ,(0.85±0.10) ratio (0.31±0.01);bFGF:(0.64±0.05),(0.53±0.02),(0.77±0.06)ratio(0.30±0.05);BDNF:(0.66±0.01),(0.58±0.05),(0.75±0.05) ratio (0.24±0.06); all P<0.05], And the Du meridian electroacupuncture rehabilitation training group was higher than Du meridian electroacupuncture group, rehabilitation training group. Conclusion In rats with spinal cord injury, Du meridian electroacupuncture and rehabilitation training can promote the functional recovery by increasing the expression of neurotrophic factor in the injured spinal cord. And there is synergy between Du meridian electroacupuncture and rehabilitation training.
