      目的 探讨创伤性脑损伤患者脑组织中miRNA let-7i水平变化及其临床意义。方法 选取金华市人民医院收治的创伤性脑损伤患者105例为病例组,分为轻度组35例、中度组56例、重度组14例; <24h组20例、≥24h且≤72h组45例、>72h组40例以及死亡组20例、存活组85例;同期选择于金华市人民医院行脑血管瘤切除术的患者76例为对照组。测定创伤性脑损伤脑组织及正常脑组织miRNA let-7i水平以及IL-4、IL-8、IL-12、IFN-γ、TNF-α、hs-CRP水平。结果 病例组miRNA let-7i水平高于对照组(P<0.05);随着脑损伤严重程度的增加,miRNA let-7i水平逐渐增加,差异有统计学意义(P<0.05);随着脑损伤时间段的延长,miRNA let-7i水平逐渐增加(P<0.05);死亡组miRNA let-7i水平高于存活组(P<0.05);病例组血清 IL-4、IL-8、IL-12水平高于对照组(P<0.05);病例组血清中TNF-α、hs-CRP均高于对照组,IFN-γ低于对照组(P<0.05);miRNA let-7i与IL-4、IL-8、IL-10、TNF-α、hs-CRP正相关关系明显(P<0.05);高水平的miRNA let-7i、GCS、TNF-α均为创伤性脑损伤患者发生死亡的危险因素(P<0.05)。结论 创伤性脑损伤患者脑组织中miRNA let-7i水平升高,其与脑损伤严重程度及脑部炎症反应密切相关;高水平的miRNA let-7i为创伤性脑损伤患者发生死亡的危险因素。

      Objective To investigate the changes of miRNA let-7i level in brain tissue of patients with traumatic brain injury and its clinical significance. Methods 105 patients with traumatic brain injury (case group) and 76 patients who underwent cerebral aneurysm resection (control group) in Jinhua People"s Hospital were selected. The case group was divided into subgroups according to the disease severity, duration of injury, and the prognosis, namely, mild group (n=35), moderate group (n=56), severe group (n=14); <24h group (n=20), ≥24h and ≤72h group (n=45),> 72h group (n=40), death group (n=20), and survival group (n=85); The levels of miRNA let-7i and IL-4, IL-8, IL-12, IFN-γ, TNF-α and hs-CRP in injured brain tissue and normal brain tissue were measured. Results The level of miRNA let-7i in the case group was higher than that in the control group (P<0.05). With the increase of the severity and duration of brain injury, the level of miRNA let-7i increased gradually (P<0.05); the level of miRNA let-7i in death group was higher than that in survival group (P<0.05); the serum IL-4, IL-8 and IL-12 levels in case group were high. Than those in the control group (P<0.05); The serum levels of TNF-α and hs-CRP in the case group were higher than those in the control group, and IFN-γ was lower than that in the control group (P<0.05).The level of miRNA let-7i was positively correlated with levels of IL-4, IL-8, IL-10, TNF-a and hs-CRP (P<0.05); High levels of miRNA let-7i, GCS and TNF-a were risk factors for death in patients with traumatic brain injury (P<0.05). Conclusion The elevated level of miRNA let-7i in brain tissue of patients with traumatic brain injury is closely related to the severity of brain injury and brain inflammation, which is a risk factor for death in patients with traumatic brain injury.
