      目的 吸气肌无力常存在于心力衰竭患者中,本研究通过超声评估膈肌功能状态,探究心力衰竭患者膈肌功能障碍对肺功能和运动耐力的影响。方法 选取66名在我院心内科住院治疗的心力衰竭患者,超声检查在吸气末端膈肌的厚度小于4.0mm为膈肌功能受损组,吸气末端膈肌厚度大于等于4.00mm为膈肌功能保留组。比较两组患者的一般特征,肺功能和呼吸肌力量,6分钟步行距离(6MWD)。结果 膈肌功能受损组患者较膈肌功能保留组的患者年龄更大(P<0.05)、左室射血分数(LVEF)更高(P<0.05),肺活量(FVC)、最大吸气压(MIP)、最大呼气压(MEP)更低(P<0.05),6MWD更短(P<0.05)。结论 通过超声评估心力衰竭患者膈肌障碍预示着肺功能差和运动耐受不良,为临床开展心力衰竭患者心脏康复提供临床依据。

      Objective Inspiratory muscle weakness is often present in patients with heart failure. SIn this study, the effect of diaphragm dysfunction on pulmonary function and exercise tolerance in patients with heart failure was investigated by ultrasonography to evaluate the functional status of diaphragm. Methods Sixty-six patients with heart failure who were hospitalized in our department of cardiology were enrolled. Ultrasound examination showed that the thickness of the diaphragm at the inspiratory end was less than 4.0 mm, which was the diaphragm dysfunction group. The thickness of the inspiratory end of the diaphragm was greater than or equal to 4.00 mm for the diaphragm function retention group. The general characteristics of the patients, pulmonary function and respiratory muscle strength, 6-minute walking distance (6MWD) were compared. Results Compared with patients in diaphragm function retention group, those in the diaphragm dysfunction group were older (P<0.05), had higher left ventricular ejection fraction (LVEF) (P<0.05), significantly lower vital capacity (FVC), lower maximal inspiratory pressure (MIP), lower maximum expiratory pressure (MEP) (P<0.05) and shorter 6MWD (P<0.05). Conclusion Ultrasound assessment of diaphragmatic disorders in patients with heart failure predicts poor pulmonary function and exercise tolerance, providing a basis for clinical cardiac rehabilitation in patients with heart failure.
