      目的 观察原发性膜性肾病(Primary membranous nephropathy,PMN)应用雷公藤多苷与环孢素A联合治疗的效果。方法 选取2015年1月~2018年1月我院52例PMN患者,采用随机数表法分为观察组(n=26)与对照组(n=26)。对照组予以环孢素A治疗,观察组给予雷公藤多苷联合环孢素A治疗。比较两组疗效、肾功能[24h尿蛋白定量(24-hour urinary protein quantity,24h Up)、尿素氮(Blood urea nitrogen,BUN)及血肌酐(Serum creatinine,Scr)]、免疫相关指标[B淋巴细胞活化因子(B lymphocyte activating factor,BAFF)、血清M型磷脂酶A2受体(Phospholipase A2 receptor,PLA2r)]、纤溶指标与内皮功能[I型纤溶酶原激活因子抑制因子(Type I plasminogen activator inhibitor,PAI-Ⅰ)、纤溶酶原激活因子(the plasminogen activator,t-PA)及内皮素-1(Endothelin-1,ET-1)]。结果 观察组总有效率高于对照组,差异有统计学意义(P<0.05);两组治疗后的24h Up水平均降低,且观察组低于对照组,差异有统计学意义(P<0.05)。两组患者后,BUN及Scr水平相比较,差异无统计学意义(P>0.05)。两组治疗后的PAI-Ⅰ、ET-1均下降,t-PA上升,且观察组PAI-Ⅰ、ET-1水平低于对照组,t-PA水平高于对照组,差异有统计学意义(P<0.05)。结论 针对PMN患者,给予雷公藤多苷联合环孢素A治疗效果确切,可促进肾功能改善,调节机体免疫,改善纤溶系统与内皮功能。

      Objective To observe the effect of combination therapy of Tripterygium Glycosides and cyclosporine A in primary membranous nephropathy. Methods A total of 52 patients with PMN who had received treatment in the Hospital from January 2015 to January 2018 were enrolled. The patients were divided into an observation group (n=26) and a control group (n=26) by random number table method. The control group was treated with cyclosporine A, and the observation group was treated with Tripterygium Glycosides combined with cyclosporine A. The efficacy, renal function indexes [24h urine protein quantitation, urea nitrogen and serum creatinine, immune related indicators] B lymphocyte activating factor, serum M-type phospholipase A2 receptor, fibrinolytic index and endothelial function indexes [type I plasmin Pro-activator inhibitor, plasminogen activator and endothelin-1] were compared between the two groups. Results The total effective rate of treatment in the observation group was shown to be higher than that in the control group (P<0.05). The 24h Up level in both groups decreased after treatment, with the level in the observation group lower than the control group, and the difference was statistically significant (P<0.05). ). There was no significant difference in the levels of BUN and Scr between the two groups (P>0.05). PAI-I and ET-1 decreased, and t-PA increased after treatment; the levels of PAI-I and ET-1 in the observation group were lower than those in the control group, and the level of t-PA was higher than that in the control group, where the difference was statistically significant (P<0.05). Conclusion For patients with PMN, Tripterygium Glycosides combined with cyclosporine A is with significant effects, which can help increase their renal function, regulate their body immunity, and improve their fibrinolytic system and endothelial function.
