      目的:探讨帕金森合并抑郁患者的临床特点及相关因素。方法:收集2016.12-2018.12本院神经内科门诊及住院的帕金森患者145例,统计帕金森合并抑郁(PD-D)的发病率,比较PD-D组和帕金森病不合并抑郁(PD-ND)组性别、年龄、发病年龄、病程、受教育程度、Hoehn-Yahr分期、帕金森评定量表III(UPDRS-III)、HAMD因子分差异,并对PD-D与相关因素(起病年龄、受教育水平、病程、H-Y分级及UPDRSIII运动评分)进行logistic回归分析。结果:145例患者中87例合并抑郁,其中轻度抑郁占75.86%;PDD组的临床特点主要表现为躯体化焦虑、强迫症状、睡眠障碍、绝望感、自卑感、认知力障碍、迟缓;与PD-ND组比较,PD-D组起病年龄轻,病程长,受教育水平低,差异具有统计学意义(P<0.05),而两组间性别、年龄差异无统计学意义(P>0.05);且相关分析显示PD-D组与病程、H-Y分级、UPDRSIII运动评分呈正相关,与起病年龄、受教育水平呈负相关,差异有统计学意义(P<0.05)。结论:帕金森合并抑郁比较常见,轻度抑郁为主,临床表现多样,与起病年龄、受教育程度、病程、H-Y分级、UPDRSIII运动评分有关,临床上应尽早干预治疗。

      ] Objective: To investigate the clinical features and related factors of patients with Parkinson's disease complicated with depression. Methods: 145 cases diagnosed as PD patients were collected in both Outpatient and inpatient neurology departments of our hospital during December 2016-December 2018, The morbidity of PD with depression (PD-D) was calculated, and the differences in gender, age, age of onset, disease duration, education level, Hoehn-Yahr stage, UPDRS III score?, and HAMD sub-factors score between the PD-D group and PD without depression (PDND) group were compared, and Logistic regression analysis of PD-D and related factors (age of onset, education level, disease duration, H-Y stage and UPDRSIII score ) was conducted. Results: 87 of the 145 patients were complicated with depression, of which mild depression accounted for 75.86%.The clinical features of PD-D group were somatic anxiety, compulsive symptoms, sleep disorder, despair, inferiority, cognitive impairment and retardation symptoms. Compared with the PD-ND group, the PD-D group had a early onset age, a long course of disease and a low level of education, and the difference was statistically significant (P < 0.05), while the gender and age differences between the two groups were not statistically significant (P > 0.05).The correlation analysis showed that PD-D group was positively correlated with the course of disease, H-Y stage, and UPDRSIII score, and negatively correlated with the age of onset and education level, with statistically significant difference (P <0.05).Conclusion: Parkinson's disease complicated with depression is relatively common, especially the mild depression, with diverse clinical features, which are related to the age of onset, education, disease duration, H-Y stage and UPDRSIII score. Therefore, early clinical intervention should be carried out.
