      目的 应用多模态超声技术定量分析药物治疗后颈动脉斑块内新生血管及斑块质地的改变,间接评估颈动脉斑块的稳定性。方法 95 例颈动脉粥样斑块患者采用随机数字表法分为治疗组和对照组,治疗组给予阿托伐他汀40mg治疗半年,对照组不给予他汀类药物治疗。所有患者治疗前后均行多模态超声技术(常规超声检查、超声造影及剪切波弹性成像)检查,并进行各参数对照比较。结果 6个月的治疗随访后,斑块最大厚度,面积较前无明显变化,治疗组和对照组比较差异无明显统计学意义(P > 0.05)。超声造影检查AUC值较前减小,剪切波弹性成像检查平均剪切波速度值较前升高,两组间比较差异有统计学意义(P < 0.05)。结论 多模态超声技术可以量化评估斑块内的新生血管及颈动脉斑块的真实质地的改变,能够更早期评估斑块稳定性,可作为临床上疗效评价的有效手段。
英文摘要:
      Objective To assess the the stability of the atherosclerotic plaque after high-dose atorvastatin therapeutic multimodal ultrasonography. Method 95 consecutive patients with carotid atherosclerotic plaques were random divided into two groups: Group A(treatment group) including 48 patients and group B (control group) including 47 patients .All patients were under the multimodal ultrasonography (conventional ultrasound, Shear wave elastography (SWE) and contrast-enhanced ultrasound) before and after treatment. Patients in group A were treated with atorvastatin (40mg) per day at least 6 months and group B weren't treated with Statin drugs, and following at least 6 months. The results of two groups were analyzed and compared. Result By treatment with atorvastatin 6 months later, compared two groups, the maximum thickness and area of the plaques has no obvious change (P>0.05); But the AUC values were significantly lower than before treatment and the SWV(SWVmean) were higher than before treaterment (P <0.05). Conclusions Contrast-enhanced ultrasound technique and shear wave elastography ,as the noninvasive approach, were able to early assessment of plaque stability.
