      目的:通过不同频率磁刺激脑梗死后吞咽功能障碍患者,观察其疗效及颅内氨基酸类神经递质的变化,探讨有效治疗频率及分子生物机制。方法:将符合纳入标准的60例研究病例,随机分为高频刺激组(10Hz)和低频刺激组(1Hz)。分别于患侧行高频刺激和低频刺激,治疗前后观察洼田饮水试验评分,吞咽造影评分和基于脑电超慢涨落理论的颅内单胺类神经递质的变化。结果:治疗前后组内比较,洼田饮水试验高频组得分升高(P≤0.05)。两组吞咽造影误吸程度得分均升高(P≤0.05),且治疗后两组组间比较,高频组得分明显高于低频组(≤0.01),差别有显著统计意义。治疗前后单胺类神经递质组内比较,高频组多巴胺水平较治疗前升高(P≤0.01),高频组兴奋性物质较治疗前升高(P≤0.05),差别有统计意义;五羟色胺,肾上腺素差别无统计意义(P>0.05)。结论:患侧高频磁刺激对脑梗死后吞咽障碍有效,其治疗分子生物学基础可能与提高吞咽皮层中枢多巴胺水平有关。

      Objective: By observing the curative effect and the influences on amino acid neurotransmitters of different frequencies of magnetic stimulation on the patients with dysphagia post stroke, to explore the effective frequencies and the molecular mechanism of magnetic stimulation on dysphagia post stroke. Methods: sixty patients who met the inclusion criteria were randomly divided into two groups: high frequency stimulation group (10HZ), and low frequency stimulation group (1HZ).The patients of two groups were separately treated with high frequency stimulation and low frequency stimulation on the affected side. The sub-water test score, swallowing imaging score and the changes of encephalic amino acid neurotransmitters based on encephalofluctuagraph therapy were observed before and after treatments. Results: The sub-water test score after treated by the high frequency stimulation group showed significantly increased than treated before (P≤0.05), the swallowing imaging score showed significantly increased after treated in two groups than treated before (P≤0.05), especially the swallowing imaging score showed significantly increased in high frequency stimulation group than in the low frequency stimulation group(P≤0.01). The level of dopamine showed increased after treated by the high frequency stimulation group showed significantly increased than treated before (P≤0.01). The level of excitatory substance showed increased after treated by the high frequency stimulation group showed significantly increased than treated before (P≤0.05).Conclusions: High frequency of magnetic stimulation on the affected side was effective on the dysphagia post stroke; the molecular mechanism may be related to increase the levels of dopamine in swallowing cortical center by high frequency of magnetic stimulation.
