| 目的 总结膝内侧MIPPO入路联合外侧入路双钢板治疗内侧粉碎股骨远端骨折中应用疗效。方法 采用膝内侧MIPPO入路联合股骨远端外侧入路、内外侧双钢板内固定治疗内侧粉碎股骨远端骨折12例，随访观察骨折愈合、并发症发生率、膝关节功能恢复情况。 结果 本组12例患者均获得随访，时间8—22个月，平均为13．5个月。 所有病例术中及术后均未出现血管神经损伤相关症状，术后无切口感染、软组织坏死及骨髓炎发生，无内固定松动断裂情况。其中1例开放性C3型骨折伴有骨缺损病例出现内翻畸形愈合，并发症发生率8.3%。参照HSS膝关节评分标准评价膝关节功能，优4例，良5例，可3例，优良率为75.0％。结论 对于内侧粉碎股骨远端骨折使用双钢板治疗能减少内固定失效、畸形愈合等并发症，而采用膝关节内侧MIPPO入路显露股骨远端内侧具有安全、创伤小、对膝关节干扰少的优点，有利于骨折愈合和膝关节功能康复。 |

| Objective To summarize the curative effect of medial knee joint MIPPO approach combined with lateral approach and double plate in the treatment of medial comminuted distal femoral fracture. Methods 12 cases of medial comminuted distal femoral fractures were treated by medial knee joint MIPPO approach combined with lateral distal femoral approach and internal fixation with internal and external double plates. The fracture healing, complications and functional recovery of knee joint were observed. Results All 12 patients were followed up for 8-22 months, with an average of 13.5 months. No symptoms related to vascular and nerve injury occurred during and after operation. No incision infection, soft tissue necrosis and osteomyelitis occurred after operation. No loosening and fracture of internal fixation occurred. One case of open type C3 fracture with bone defect had varus malunion, and the incidence of complications was 8.3%. According to HSS knee scoring criteria, knee function was evaluated in 4 cases with excellent results, 5 cases with good results and 3 cases with fair results. The excellent and good rate was 75.0%. Conclusion For medial comminuted distal femoral fractures, double plate treatment can reduce the complications of internal fixation failure、 malunion and other complications. The MIPPO approach to reveal medial femoral fractures through medial knee joint has the advantages of safety,、less trauma and less interference to the knee joint, which is conducive to fracture healing and functional rehabilitation of the knee joint. |
