| 摘要 目的:探讨肠梗阻导管联合清胰汤治疗重症急性胰腺炎的疗效分析，方法 选取我院诊治的重症胰腺炎患者共60例，分为对照组30例和治疗组30例，对照组实施常规胃管减压联合清胰汤、治疗组患者实施肠梗阻导管减压联合清胰汤。观察比较两组患者的腹痛、腹胀、肛门排气等临床症状改善情况，同时评价治疗7天后APACHEⅡ评分、C-反应蛋白（CRP）的变化情况，动态测量腹腔内压力及减压引流量及计算15天出院率情况。结果 治疗组在腹痛腹胀消失、肛门排气恢复时间均明显提前于对照组，有统计学意义； 治疗7d 后，两组血清指标变化情况对比，APACHE Ⅱ评分、血清CRP均较治疗前降低，且治疗组较对照组降低幅度差异在统计学上具有意义（P＜0.05）；治疗后24h，48h，72h的腹腔压力，治疗组较对照组腹腔内压力降低幅度明显大于对照组，腹腔内压力水平和对照组有差异比较有统计学意义；治疗后前3天日均引流量治疗组较对照组明显增多，具有统计学意义；住院15天出院率，治疗组较对照组亦明显上升。结论：肠梗阻导管联合清胰汤治疗重症急性胰腺炎效果确切，可以早期改善胰腺炎主要临床症状，有效降低腹腔压力，快速恢复肠功能障碍，值得临床推广应用。 |

| 【Abstract】Objective To investigate the effects of Ileus tube(IT) combined with Qingyi decoction in the treatment of severe acute pancreatitis.Methods Sixty patients with SAP were randomly divided into two groups, nasogastric decompression tube with Qingyi decoction was used in the control group(n=30). Ileus tube(IT) combined with Qingyi decoction was used in the treatmentl group(n=30),The sustained time of abdominal pain,the time of relief of abdominal distensio,the time of bowel sound recovery were observed，Before the treatment and 7 days after the treatment，the acute physiology and chronic health evaluation I(APACHE I)score of the patient health condition and the level of C reaction protein in the two groups were assessed,At the same time,dynamic changes in decompression drainage and differences in the IAP,the average length of hospitalization also were recorded.Results In the treatment group，the time of abdominal pain relief being days,the amelioration time of abdominal distention days，the time of bowel sound recovery days were shorter than those in the control group with statistically significant differences.Seven days after the treatment，the score of APACHE 1 in the treatment group and the level of C reaction protein were significantly lower than that of the control group,the change of the IAP was significantly lower than that of the control group with statistically significant difference;Within 72 hours of treatment，The average volume of of drainage in the treatment group was more than that of the control group;the average length of hospitalization(15 days) is also much shorter. Conclusion：the treatment of Ileus tube(IT) combined with Qingyi decoction has good therapeutic effects in patients with severe acute pancreatitis, it can earlier relieve clinical symptoms, gastrointestinal dysfunction can be effectly improved,also it can significantly reduce the Intra-abdominal pressure,this technique should be recommended. |
