      摘要 目的:观察并探讨丙泊酚靶控输注下全麻诱导喉罩置入时无体动反应时的盐酸羟考酮的半数有效浓度(ECe50)。方法:选择本院30例择期行大隐静脉手术患者,ASAⅠ~Ⅱ级,不使用肌松药麻醉诱导,盐酸羟考酮的剂量通过改良的Dixon序贯法确定,用Marsh药代动力学模型以血浆靶浓度4.0μg/ml输注丙泊酚,注射盐酸羟考酮5min后置入喉罩,计算出所需盐酸羟考酮的半数有效剂量(ECe50)。观察并记录患者基础值(T0)、 给药1min(T1)、3min(T2)、5min(T3)、喉罩置入即刻(T4)、及喉罩置入后1min(T5)各时期的平均动脉压(MAP)、心率(HR)、手术体积指数(SPI)及脑电双频谱指数(BIS)等指标,观察患者术后不良反应。结果:丙泊酚靶控输注全麻诱导喉罩置入时盐酸羟考酮的半数有效浓度为0.08mg/kg,其95%可信区间为(0.02-0.11)mg/kg(P<0.01),与T0比较T3、T4时间点的MAP及T4、T5时间点的HR均明显下降(P<0.05);T0 与T4 时间点的BIS、SPI值均明显下降(P<0.05),术后随访患者均无麻醉诱导期不良记忆。结论:丙泊酚靶控输注不使用肌松药全麻诱导喉罩置入时无体动反应时的盐酸羟考酮的半数有效浓度为0.08mg/kg,低于盐酸羟考酮气管导管插管的有效浓度0.30mg/kg,可应用于快通道麻醉喉罩置入,方便日间手术患者的气道管理。

      【Abstract】Objective To observation and explore the median effective effect-site concentration (ECe50) of oxycodone hydrochloride for laryngeal mask airway(LMA) insertion with no body movement during anesthesia induction with target-controlled infusion(TCI) of propfol. Methods 30 ASAⅠ-Ⅱ patients on saphenous vein extration surgery were selected for this study. According to reformative Dixon's up-and-down injection methodology, Oxycodone hydrochloride was injected while propfol TCI(4.0ug/ml) of Marsh pharmacokinetic model was performing without using muscle relaxants , and LMA insertion was carried out 5 minutes after the injection of oxycodone hydrochloride.Then the ECe50 of oxycodone hydrochloride was culculated with Probit analysis. Results The ECe50 of oxycodone hydrochloride for LMA insertion during anesthesia induction with TCI of propfol is 0.08mg/kg. Conclusion Without muscle relaxants, the ECe50 of oxycodone hydrochloride for LMA insertion during anesthesia induction with TCI of propfol is 0.08mg/kg , 0.3mg/kg lower than that for tracheal intubation, which makes no patient respond. So that LMA insertion can be carried out at an effective concentration to provide convenience of airway management for day surgery .
