      目的 探讨正念干预训练在改善青少年PTSD患者的病情以及睡眠、焦虑、抑郁等情绪障碍方面的临床效果。方法 选取2017年1月-2017年12月于我院就诊的PTSD患者共40名,其中男性12名,女性28名。将受试者随机分为实验组和对照组,每组20名。对照组进行传统的以创伤为中心的认知行为疗法,实验组接受正念干预训练。患者入组后每周接受2次集中访谈和训练,共持续8周。实验前和干预训练8周后分别检测患者各量表的得分,比较和评价不同干预训练的临床效果。结果 8周的干预训练后,两组患者的PTSD量表得分均显著降低(所有P<0.001),且两组患者干预后的得分差异无统计学意义。实验组患者干预前后PSQI得分、PHQ-9得分和GAD-7得分均显著下降,差异具有统计学意义(所有P<0.05),而对照组仅PSQI得分和GAD-7得分的下降差异显著(二者P<0.05),PHQ-9得分在认知行为疗法治疗前后无明显变化(p>0.05)。同时,实验组PSQI得分和PHQ-9得分在干预训练后均显著低于对照组。结论 8周的正念干预训练显著改善了青少年患者的PTSD病情以及睡眠质量、焦虑、抑郁等情绪障碍。与传统的以创伤为中心的认知行为疗法相比,正念干预训练在改善睡眠质量和抑郁情绪方面效果更显著。

      Objective To explore the clinical effects of mindfulness intervention in improving the disease condition of adolescent PTSD patients and their emotional disorders such as sleep disorders, anxiety and depression. Methods A total of 40 PTSD patients were enrolled in our hospital from January 2017 to December 2017, including 12 males and 28 females. The subjects were randomly divided into experimental group and a control group, with 20 patients in each group. The control group underwent trauma-focused cognitive behavioral therapy, and the experimental group received mindfulness intervention training. The patients received 2 concentrated interviews and trainings per week for 8 weeks. The scores of each scale of the patients were collected before the experiment and after 8 weeks of intervention, and the clinical effects of different interventions were compared and evaluated. Results After 8 weeks of intervention, the PTSD scale scores of both groups were significantly reduced (all P < 0.001), and there was no significant difference in the scores between the two groups after intervention. The PSQI score, PHQ-9 score and GAD-7 score of the experimental group were decreased after intervention, the differences were statistically significant (all P<0.05). The control group only had significant decreases in the PSQI score and the GAD-7 score after intervention (P<0.05), the PHQ-9 scores in the control group did not change significantly after cognitive behavioral therapy (p>0.05). Meanwhile, the PSQI score and PHQ-9 score of the experimental group were significantly lowered than those of the control group after the intervention training. Conclusion Eight weeks of mindfulness intervention training significantly improved the PTSD conditions and emotional disorders such as sleep quality, anxiety and depression in adolescents. Compared with the traditional trauma-focused cognitive behavioral therapy, mindfulness intervention training is more effective in improving sleep quality and depression.
