      目的:探讨益气活血利湿汤对糖尿病视网膜病变大鼠炎症因子及视神经保护作用的影响。方法:10只SD大鼠为对照组,40只SD大鼠通过腹腔注射链脲佐菌素和玻璃体腔注射VEGF建立糖尿病视网膜病变大鼠模型,随机分为模型组,益气活血利湿汤高中低剂量组,每组10只,模型对照组和空白对照组给予蒸馏水灌胃,益气活血利湿汤高中低剂量组给予中药煎煮液灌胃。治疗8周后观察视网膜和视神经结构,采用ELISA检测血清肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)和白细胞介素-6(IL-6)水平,RT-PCR检测视网膜神经生长因子(NGF)和核因子-κB(NF-κB) mRNA表达。结果:模型组视网膜出现水肿和扩张充血毛细血管;低剂量组视网膜水肿,结构紊乱,血毛细血管扩张充,有新生血管;中剂量组视网膜结构相对完整,水肿明显减轻;高剂量组视网膜结构基本完整,未见明显水肿、毛细血管扩张充血和新生血管。模型组神经纤维排列紊乱,轴突重度肿胀,多处毛细血管扩张出血,胶质细胞增生;低剂量组视神经纤维排列不规则,轴突肿胀减轻;中剂量组视神经纤维轴突肿胀明显减轻,偶见毛细血管扩张出血和胶质细胞增生;高剂量组视神经纤维排列整齐规则,结构较致密,未见毛细血管扩张出血和胶质细胞增生。模型对照组血清TNF-α、hs-CRP和IL-6水平明显高于空白对照组,益气活血利湿汤高中低剂量组血清TNF-α、hs-CRP和IL-6水平低于模型对照组,但高于空白对照组,随益气活血利湿汤剂量增加,TNF-α、hs-CRP和IL-6水平降低(P<0.05)。与对照组比较,模型组视网膜NGF和NF-κB mRNA表达下调,益气活血利湿汤灌胃后视网膜NGF和NF-κB mRNA表达上调,益气活血利湿汤剂量越大NGF和NF-κB mRNA表达越高(P<0.05)。结论:益气活血利湿汤治疗糖尿病视网膜病变大鼠,可改善视网膜和视神经结构,减轻炎症反应,上调NGF和NF-κB表达。

      Objective: To study the influence of Yiqi huoxue lishi decoction on inflammatory factor and optic nerve protection in diabetic retinopathy rat. Methods: Ten SD rats were selected as control group, and forty SD rats were established diabetic retinopathy model by intraperitoneal injection of urea with cephalosporins and VEGF glass body cavity injection, and were divided into model group, yiqi huoxue lishi decoction high middle and low dose group randomly, 10 each group. The control group and model control were given distilled water by gavage, the yiqi huoxue lishi decoction high, middle and low dose group were given traditional Chinese medicine decoction by gavage. After 8 weeks treatment, the structure of retina and optic nerve were observed, the levels of serum tumor necrosis factor alpha (TNF-α), hypersensitive c-reactive protein (hs-CRP) and interleukin 6 (IL-6) were detected by ELISA, the expression of retinal nerve growth factor (NGF) and nuclear factor kappa B (NF-κB) mRNA were detected by RT-PCR. Results: The model group appeared retinal edema and congestion capillaries expansion, the low dose group appeared retinal edema, structure disorder, blood capillary dilate, filling with new blood vessels, middle dose group had relatively retinal structure, obviously relieving edema, high dose group had basic integrity retinal structure, no obvious edema, hyperemia and capillaries expansion of new blood vessels. The model group disordered arrangement of nerve fibers, severe axons swelling, capillary expansion bleeding, glial cell proliferation, low dose group had irregular optic fiber arrangement, axons swelling reducing, middle dose group had significantly reducing optic fiber axon swelling, occasional expansion of capillary hemorrhage and glial cell proliferation, high dose group had optic fiber order rules, compact structure, no expansion of capillary hemorrhage and glial cell proliferation. The levels of serum TNF-α, hs-CRP and IL-6 in model control group were significantly higher than the control group, the levels of serum TNF-α, hs-CRP and IL-6 in high, middle and low dose group were lower than the model group, but with dose of yiqi huoxue lishi decoction increasing, the levels of serum TNF-α, hs-CRP and IL-6 were decreased (P<0.05). Compared with control group, the expression of NGF and NF-κB mRNA in model group were lower, the expression of NGF and NF-κB mRNA in high, middle and low dose group were higher, the more high dose of yiqi huoxue lishi decoction, the more higher expression of NGF and NF-κB mRNA(P<0.05). Conclusion: Yiqi huoxue lishi decoction on treatment of diabetic retinopathy rat can improve the structure of retina and optic nerve, reduce inflammation, raising expression of NGF and NF-κB.
