| 目的：评价经皮质骨轨迹（Cortical Bone Trajectory，CBT）联合传统椎弓根轨迹（Traditional Trajectory，TT）置钉固定技术在治疗老年腰椎结核中的临床效果。方法：回顾性分析2014年1月~2016年12月在我院行一期后路CBT联合TT内固定+前路病灶清除+取自体髂骨植骨融合术的32例老年腰椎结核患者，其中男性17例，女性15例。研究中对比分析术前及术后的临床表现、实验室检查及影像学检查，以评估疗效。结果：研究平均获得随访时间15.6个月；至末次随访，32例患者结核中毒症状均不明显，VAS评分由（5.0±0.7）降至（2.3±0.4）分，Frankel分级得到明显改善，ESR、CRP恢复正常，Cobb角由（17.8±5.6）°降至（4.8±2.2）°，随访期内Cobb角丢失小，内固定无失效，固定牢靠。结论：经皮质骨轨迹联合传统椎弓根轨迹置钉固定技术是一种适合老年腰椎结核的手术内固定方式。 |

| Abstract Objective: To evaluate the clinical effect of Cortical Bone Trajectory (CBT) combined with traditional Trajectory (TT) fixation in the treatment of elderly lumbar tuberculosis.Methods: Retrospective analysis of 32 cases of elderly lumbar tuberculosis patients with anterior approach CBT combined with TT internal fixation + anterior debridement + autogenous iliac bone grafting from January 2014 to December 2016, including 17 male and 15 women .The clinical performance, laboratory examination and imaging examination before and after surgery were compared and analyzed to evaluate the efficacy.Results: The average follow-up time was 15.6 months. At the last follow-up, the symptoms of tuberculosis were not obvious in 32 patients. The VAS score decreased from (5.0±0.7) to (2.3±0.4), the Frankel grade improved significantly, the ESR and CRP returned to normal, and the Cobb angle was (17.8± 5.6) ° fell to (4.8 ± 2.2) °, the Cobb angle loss was small during the follow-up period, the internal fixation did not fail, and the fixation was firm.Conclusion：Cortical bone trajectory combined with traditional pedicle trajectory fixation is a surgical fixation method for elderly lumbar tuberculosis. |
