      摘要 目的 研究右归饮联合富血小板血浆(PRP)对去卵巢骨质疏松骨折大鼠影像学、骨密度和生物力学的影响。方法 切除卵巢雌性大鼠,分为正常对照组、模型组,模型组分为右归饮治疗组、PRP治疗组、右归饮联合PRP治疗组,正常对照组不做处理,其中PRP治疗组和右归饮联合PRP治疗组大鼠于骨折造模术前眼眶采血3mL,制备PRP,在大鼠双侧胫骨近端骨折模型建立术中将预先制备好的PRP凝胶注入骨折端。右归饮治疗组及右归饮联合PRP治疗组给予右归饮灌胃,骨折术后第1、2、4、6周分批处死动物。观察双侧胫骨骨折端影像学、骨痂骨密度(BMD)、生物力学,比较四组大鼠骨折愈合情况。结果 从X线可见,在术后第6周,正常对照组和右归饮组骨折线非常模糊,隐约可见,PRP组和联合组大部分骨折端骨折线消失,骨折断端、髓内骨性骨痂较第4周密度更高,骨痂塑型较好,PRP组和联合组骨折处骨痂塑形基本完成,髓腔基本贯通;从Micro-CT的骨痂图中可见,右归饮组、PRP组及联合组大鼠在骨折术后第1、2、4周骨痂的骨量较正常对照组高,第4周时PRP组及联合组骨痂骨量明显高于正常对照组和右归饮组;从Micro-CT软件的定量分析结果中可见,联合组、PRP组、右归饮组骨性骨痂体积(BV)及骨性骨痂和总骨量的体积比(BV/TV)在骨折术后第1、2、4周均高于正常对照组,每个时间点都是联合组BV最高,其中术后第1、2、4周组间两两比较有统计学差异(p<0.05);从生物力学分析图可见,在术后第4、6周,右归饮组、PRP组及联合组大鼠的骨痂最大扭力值和硬度值均显著高于正常对照组(p<0.05),且联合组与右归饮组、PRP组比较有统计学差异(p<0.05),在术后第2周,四组比较无统计学差异。结论 右归饮联合PRP对去卵巢骨质疏松骨折大鼠的疗效较单纯的右归饮和PRP治疗效果更佳,能显著提高骨折愈合质量,明显缩短骨折愈合周期。 关键词 骨质疏松;影像学;骨密度;生物力学;大鼠

      Effects of You Gui Yin and Platelet Rich Plasma on the imaging, bone mineral density and biomechanics of ovariectomized osteoporosis rats Gao-guowei,Zhao-lilai*,Tong-peijian,Ruan-wenbin,Zhang-xuechao, *People's Hospital of Anji Country, Anji 313300 ,Zhejiang, China ABSTRCT Objective:To investigate the effects of You Gui Yin and Platelet Rich Plasma on the imaging, bone mineral density and biomechanics of ovariectomized osteoporosis rats .Methods:Female ovariectomized rats were divided into normal control group and model group, and the model group was divided into You Gui Yin group, PRP group, You Gui Yin +PRP group. 3 ml blood was collected before surgery of fracture to prepare PRP by the method of Landsberg. The PRP was injected to the site of fracture during the operation in PRP and You Gui Yin +PRP group. You Gui Yin was administered by gavage daily to You Gui Yin group and You Gui Yin +PRP group .Specimens were harvested postoperatively at 1, 2, 4 and 6 week for X-ray, micro-CT, BMD, biomechanical testing . Results:From X-ray visible and 6 weeks after surgery, the normal control group and You Gui Yin group of fracture line was fuzzy, faintly visible, PRP group and You Gui Yin +PRP group most of the fracture end fracture line disappeared, fracture end, intramedullary bone callus is more sophisticated than the 4th, callus plastic is better, PRP group and You Gui Yin +PRP group fracture callus plastic is basically completed, the pulp cavity is basically through.The precision of the result of quantitative analysis of the Micro - CT software of visible, the You Gui Yin +PRP group, the PRP group, the You Gui Yin group bony callus volume (BV) and bony callus and the volume of total bone mass ratio (BV/TV) in 1, 2, 4 weeks after fracture is higher than the normal control group, each point in time is the highes the You Gui Yin +PRP group BV, among them 1, 2, 4 weeks after compared the was statistically difference between two groups (p < 0.05).As can be seen from the biomechanical analysis figure, the maximum torsional force and hardness of callus of rats in the You Gui Yin group, PRP group and You Gui Yin +PRP group were significantly higher than those in the normal control group at the 4th and 6th weeks after surgery (p<0.05), and there were statistically significant differences between the You Gui Yin +PRP group, the You Gui Yin group and the PRP group (p<0.05), while there were no statistically significant differences between the four groups at the 2th week after surgery.Conclusion: The effect of the You Gui Yin in Combination with PRP was better than You Gui Yin or PRP for osteoporotic fracture. It can obviously promote the growth of callus and shorten fracture healing process(p<0.05). KEYWORDS Osteoporosis;Imaging;Bone mineral density; Biomechanics;Rat
