      抗生素相关性腹泻(Antibiotic-associated diarrhea , AAD) 是指应用抗生素后发生的、与抗生素有关的腹泻[1]。近年来,儿科临床抗生素滥用现象非常严重[2]。抗生素在杀灭病原菌的同时,也对患者肠道微生态系统造成了破坏。有报道称AAD的发生率达5%~39%[3]。临床研究显示[4],微生态制剂能够降低AAD的发生率,但其作用机制尚不完全清楚。殷朝阳等[5]研究显示,AAD中血清α肿瘤坏死因子(tumor necrosis factors,TNF-α)、白介素-6(Interleukin-6,IL-6)显著升高。本研究通过检测AAD患者血清TNF-α、 IL-6浓度变化,探讨TNF-α、IL-6在AAD中的变化及布拉氏酵母菌散剂对其影响。
英文摘要:
      Objective To investigate the effects of boulardii on infant with antibiotic associated diarrhea and influence of which on cell factor in serum. Methods 70 cases were diagnosed as children with antibiotic associated diarrhea in the Comprehensive Ward of Hangzhou children"s Hospital from March 2015 to March 2016, which were randomly divided into the control group and the observation group according to random digit, with 35 cases each. The control group were treated according to primary disease, and the observation group were treated with oral boulardii on the basis of the control group. Then the frequency of stool was compared between the 2 groups. the levels of TNF-α and IL-6 in serum were detected and the correlation analysis were analysed. Results On the fifth day after treatment, the number of diarrhea in the treatment group was significantly lower than that in the control group (t=2.45, p<0.05). while on the tenth day after treatment the diarrhea times in the treatment group were significantly more lower than which in the control group (t=4.28, p<0.01);The duration of diarrhea was compared, on the fifth day after treatment the treatment group was significantly less than the control group(t=4.28,p<0.01). the level of TNF-α and IL-6 were compared,which were significantly lower in the treatment group than in the control group ( t=2.15;3.27,P<0.05); while on the tenth day after treatment,which were significantly lower in the treatment group than in the control group (t=6.18;3.45,P<0.01);and the concentration of serum TNF-α and IL-6 were positively correlated with the time of treatment of Boulardii (r=0.92;0.88,p <0.01), and the serum TNF-αlevels were positively correlated with IL-6 (r=0.80,p <0.01). Conclusion Boulardii can reduce the incidence of antibiotic associated diarrhea, which can significantly reduce levels of TNF alpha and IL-6 in serum .
