| 目的 系统评价中医及中西医联合治疗哺乳期乳腺炎疗效的有效性。方法 计算机检索国内期刊数据库（中国知网、万方期刊全文数据库和维普中文科技期刊数据库），搜索中医及中西医联合治疗哺乳期乳腺炎的随机对照研究，检索时限为2000年1月至2017年12月。由两位研究者按照纳入与排除标准独立进行文献筛选、资料提取和偏倚风险评价，采用RevMan 5.3软件进行Meta分析。结果 纳入31个随机对照研究，共4013例患者，其中干预组和对照组分别为2224例和1789例；纳入文献整体质量偏低，存在明显的发表偏倚。Meta分析结果显示：与单纯西医治疗相比，中医及中西医联合治疗会提高哺乳期乳腺炎的临床疗效[RR=1.21，95%CI（1.16～1.27），P＜0.01]；按照干预方法进行分层，分析结果显示中药内服治疗、中药内服+其他中医疗法（中药外敷、针刺、手法疏通）、中西医联合治疗相比于单纯西医治疗均具有一定优势。结论 当前临床证据表明，中医及中西医联合治疗哺乳期乳腺炎的疗效要优于单纯西医治疗。然而，受限于纳入研究的质量和干预方法的多样性，上述结论尚需进一步开展更多高质量大样本的研究予以验证。 |

| Objective To systematically review the effectiveness of traditional Chinese medicine and the combined Chinese and Western medicine for lactational mastitis. Methods We electronically searched databases including CNKI, Wanfang Data databases for published articles of randomized controlled trials (RCTs) of traditional Chinese medicine and the combined Chinese and Western medicine for lactational mastitis from January, 2000 to December, 2017. According to the inclusion and exclusion criteria, at least two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies. Meta-analysis was performed using RevMan 5.3 software. Results A total of 31 RCTs involving 4 013 patients ( intervention group: 2 224 patients, control group: 1 789 patients ) with lactational mastitis were include. The overall quality of 31 literatures was less than satisfactory. Funnel plot graphics indicated publication bias. The results of meta-analysis showed that the effectiveness of traditional Chinese medicine and the combined Chinese and Western medicine was superior to western medicine alone [RR=1.21，95%CI（1.16~1.27），P＜0.01]. The stratified analysis by therapies showed that the effectiveness of Chinese herbals, Chinese herbals and other traditional Chinese therapies ( external application of Chinese medicine, acupuncture treatment, manipulation cheirapsis ) were superior to western medicine alone. Conclusion The current evidence shows that traditional Chinese medicine and the combined Chinese and Western medicine may significantly improve the effectiveness. However, due to the generally low methodological quality and the variations of the therapies used, the above conclusions need more high quality and large-scale sample studies to verify. |
