      目的 探讨白芨水煎剂对结肠黑变病(melanosis coli,MC)豚鼠结肠黏膜上皮细胞凋亡和凋亡相关蛋白TNFα、Bcl-2和Bax表达的影响。方法 清洁级豚鼠40只,完全随机分为3组:正常组8只,模型组16 只和白芨水煎剂组16只。模型组及白芨剂组均予大黄提取液6g/kg·d连续灌胃60d。造模开始2周后,模型组予生理盐水灌肠,白芨组予10%白芨水煎剂灌肠,每周3次。造模结束后,观察豚鼠肠壁色素沉着情况,行结肠组织HE染色和黑色素染色。应用TUNEL染色检测结肠上皮细胞凋亡,Western blotting检测TNFα、Bcl-2和Bax的表达。结果 与模型组(4.81±0.40)相比较,白芨组豚鼠的结肠黑变病评分(2.88±0.34)显著降低(p<0.05),结肠黏膜上皮细胞TUNNEL染色细胞凋亡阳性率(16.97±1.46%)显著低于模型组(23.28±1.94%)(p<0.01)。与正常组相比较,模型组豚鼠结肠组织TNFα和Bcl-2的蛋白表达量显著增高,分别为正常组的4.27倍和3.68倍(均p<0.001)。白芨组豚鼠结肠的TNFα和Bcl-2表达量较模型组显著下降,分别为模型组的54.6%和47.8%(均p<0.001)。相反,模型组豚鼠结肠组织中Bax的表达量与正常组相比显著下降,为正常组的45.0%(p<0.001)。白芨组的Bax表达比模型组显著增高,为模型组的1.96倍(p<0.001)。结论 白芨水煎剂可能通过抑制MC豚鼠结肠组织TNFα和Bcl-2的蛋白表达,增加Bax表达,降低Bcl-2/Bax的比值,抑制结肠上皮细胞凋亡,从而改善MC。

      Objective To investigate the effects of bletilla decoction on apoptosis of colonic mucosal epithelial cell and expressions of apoptosis related protein TNFα, Bcl-2 and Bax in guinea pig’s colon of MC. Methods 40 guinea pigs were randomly divided into 3 groups: control group (n=8), model group (n=16), bletilla group (n=16). The model group and the bletilla group were induced by intragastric administration of rhubarb at a dose of 6g/kg·d for 60 consecutive days. After 2 weeks, the animals in the bletilla group were treated with 10% bletilla decoction enema for three times a week while the model group were treated with NS enema. The pigmentation of intestinal wall was observed after model preparation, while the intestinal tissue was for HE staining and melanin staining. Apoptosis of epithelial cells was detected TUNEL staining. The expressions of TNFα, Bcl-2 and Bax in colon were detected by western blotting. Results Compared with the model group(4.81±0.40), the colonic melanosis score in guinea pig of bletilla group(2.88±0.34)was significant lower than the model group(p<0.05), and the colonic mucosal epithelial cell TUNEL staining of bletilla group(16.97±1.46%)was significantly lower than that in model group(23.28±1.94%)(p<0.01). Compared with the control group, the protein expression of TNFα and Bcl-2 of colonic tissue in model group was significantly increased, which were 4.27 times and 3.68 times of the normal group respectively (both p<0.001). And those in bletilla group was significantly lower than model group, which were 54.6% and 47.8% of the model group respectively (both p<0.001). Contrary, the protein expression of Bax of colonic tissue in model group was significantly lower than control group, which was 45.0% of the control group (p<0.001). The protein expression of Bax in bletilla group was significantly increased than the model group, which was 1.96 times of the model group (p<0.001). Conclusion Bletilla decoction may improve MC by inhibiting the expression of TNFα and Bcl-2, increasing the Bax expression, decreasing the ratio of Bcl-2/Bax, and inhibiting the apoptosis of intestinal epithelial cell.
