      目的 探讨急性颅脑损伤患者外周血中辅助性T细胞17(Th17)和调节性T细胞(Treg)及其相关细胞因子白介素17(IL-17)、白介素23(IL-23)、 转化生长因子(TGF-β1)的变化及临床意义。方法 选取2015年1月至2016年12月住院治疗的104例急性颅脑损伤患者为研究对象,根据格拉斯哥昏迷量表(GCS)评分分组,轻型颅脑损伤组(A组)34例,中型颅脑损伤组(B组)38例,重型颅脑损伤组(C组)32例。另选取同期行健康体检的志愿者30例作为健康对照组(D组)。受伤后次日清晨分别采集各组的外周静脉血,采用ELISA检测IL-17、IL-23、 TGF-β1水平;采用流式细胞术检测Th17细胞和Treg细胞的比例。结果 各组急性颅脑损伤患者血清中的Th17细胞、IL-17及IL-23均显著高于健康对照组(P<0.05),且重型颅脑损伤组显著高于轻型和中型颅脑损伤组(P<0.05),中型颅脑损伤组显著高于轻型颅脑损伤组(P<0.05);中重型颅脑损伤患者血清中Treg细胞及TGF-β1均显著低于健康对照组(P<0.05),重型颅脑损伤组显著低于轻型和中型颅脑损伤组(P<0.05),中型颅脑损伤组显著低于轻型颅脑损伤组(P<0.05)。与对照组比较,轻型颅脑损伤患者血清中Treg细胞及TGF-β1水平降低不明显(P>0.05)。结论 Th17/Treg细胞失衡参与急性颅脑损伤后免疫炎性反应过程,Th17过亢及Treg不足可能与急性颅脑损伤后的脑损伤程度相关,检测Th17/Treg细胞及其相关细胞因子水平有助于评估急性颅脑损伤患者的病情。

      Objective To investigate the expression of helper T cells 17(Th17 cells) and regulary T cells (Treg cells) and their related cytokines in the patients with acute brain trauma and to explore its clinical significance further. Methods According to the results of Glasgow Coma Scale, 34 patients with mild traumatic brain injury (group A),38 patients with moderate traumatic brain injury(group B) and 32 patients with severe traumatic brain injury(group C) hospitalized in Hangzhou Traditional Chinese Medical Hospital from January 2015 to December 2016 were selected, and 30 healthy people underwent physical examination in this hospital at the same period were enrolled as the control group(group D).The proportion of Th17 cells and Treg cells in the peripheral blood were detected by flow cytometry instrument,and the related cytokines including interleukin 17(IL-17),IL-23 and TGF-β1 were measured by Enzyme-linked immunosorbentassay (ELISA) in plasma of patients with acute brain trauma. Results Compared with the control group, The ratio of Th17 cells and the levels of IL-17,IL-23 in all acute brain trauma groups were significantly increased (P<0.05). The ratio of Th17 cells and the levels of IL-17,IL-23 in the Group C were significantly higher than those in the Group A and Group B (P<0.05),and those in the Group B were significantly higher than those in Group A (P<0.05).The ratio of Treg cells and the levels of TGF-β1 in Group B and Group C were lower than those in control group (P<0.05), and those in the Group C were significantly lower than those in the Group B and Group A (P<0.05), and those in the Group B were significantly lower than those in the Group A (P<0.05). Compared with Group D,The ratio of Treg cells and the levels of TGF-β1 in Group A had no statistically significant difference (P>0.05). Conclusion The imbalance of Th17/Treg cells are involved in the progress of inflammatory response after acute brain trauma. Excessive Th17 cells and insufficient Treg cells are related with brain injury after acute brain trauma.Evaluation of the Th17/Treg balance and the cytokine levels can help to assess the severity of patients with acute brain trauma.
