| 【摘要】：目的 探讨原发性甲状腺淋巴瘤的临床表现及诊治。方法 回顾性分析2012年至2017年浙江中医药大学附属第一医院收治的3例原发性甲状腺淋巴瘤病例资料，分析其临床表现、诊断和治疗。结果 女性2例，男性1例，平均年龄71岁。临床表现均为无痛性甲状腺肿块，伴有不同程度的压迫症状。实验室检查无特异性指标，2例伴有桥本氏甲状腺炎；术前均未明确诊断，本组3例均行甲状腺腺叶切除或次全术加术中冰冻及病理切片;病理均为甲状腺弥漫性大B细胞淋巴瘤。术后均化疗，随访12～60月，随访期间1例死亡，2例存活。结论 原发性甲状腺是罕见的恶性肿瘤，侵袭力强，误诊率高，手术联合放化疗的综合治疗模式有望提高患者生活质量及延长生存时间。 |

| To analyze the clinical manifestation and diagnosis of primary thyroid lymphoma. Methods Three cases with primary thyroid lymphoma who were admitted between 2012 and 2017 to The First Affiliated Hospital of Zhejiang Chinese Medical University were retrospectively analyzed. Its clinical manifestation and diagnosis were also analyzed. Results Two patients were female and the other was male，with an average age of 59 years. Their clinical manifestations were painless tumor with different degrees of compression symptoms. The laboratory examination of the disease showed no specific indicator.Two cases have Hashimoto's thyroiditis.?No definite diagnosis was made before operation.3 patients underwent thyroidectomy or Subtotal thyroidectomy ,intraoperative frozen and pathological sections. In the study，the pathological types were thyroid diffuse large B-cell lymphoma. 3 patients received chemotherapy after surgery.Three patients were followed up for 12 to 60 months.During the follow-up, 1 patient died and 2 survived. Conclusion PTL are rare malignant tumors with highly invasive, high?misdiagnosis?rate and poor prognosis.Surgery combined with radiotherapy and chemotherapy may ,to a certain extent，improved quality of life and prolong survival time. |
