      目的:探讨甲状腺激素水平与轻度认知功能障碍(Mild cognitive impairment,MCI)脾肾两虚证的相关性,为防治MCI提供依据。方法:应用病例对照研究方法,纳入43例MCI患者、48例认知功能正常老人,收集四诊资料及MCI发病的一般情况,检测甲状腺功能,对MCI组、对照组对照研究,同时分析MCI患者的中医基本证候。结果1. 对照组(P<0.05)的TSH水平明显高于MCI组,其余甲状腺功能指标无显著性差异(P<0.05),两组其它甲功指标的差异无显著性意义。2. MCI患者的中医证候分别为脾肾两虚型(41.86%)、气阴两虚型(30.23%)、肝肾阴虚型(13.95%)、痰湿阻络型(9.30%)、湿热壅滞型(4.65%)。结论:低TSH水平可能是MCI发病的危险因素;MCI的中医证候以脾肾两虚型多见;甲状腺激素水平与脾肾两虚证无相关性。

      Objective: To explore the correlation between thyroid hormone levels and and mild cognitive impairment (MCI), and TCM spleen- kidney deficiency syndrome through case-control study, providing reference for the further prevention of MCI and TCM syndrome differentiation of abnormal thyroid hormone levels. Methods: Draw up MCI clinical observation table, then investigate the inpatients and outpatients, 43 MCI patients and 48 normal cognitive function of the elderly individuals aged older than 70 years were included. Make TCM diagnostic data collection of all objects as well as the general pathogenesis situation of MCI, and five indicators of thyroid function of all individuals were also collected, including the total three-triiodothyronine (TT3), total thyroxine (TT4), thyroid-stimulating hormone (TSH), free three triiodothyronine (FT3) and free thyroxine (FT4). Finally establish the database, use the 1:1 case-control study to explore the correlation between abnormal thyroid hormone levels and MCI, the distribution of TCM basic symptom in MCI and the correlation between thyroid hormone levels and spleen-kidney deficiency syndrome, and the characters of neuropsychological testing in MCI as well as the general clinical features of MCI are also discussed through statistical analysis. Results: 1. The 1:1 case-control study shows that the TSH level of MCI group is lower than that of the control group (P<0.05), but there is not significant difference of other indicators of thyroid function between MCI group and control group. 2. The study of TCM syndrome indicates that the spleen-kidney deficiency type is the most common syndrome in all five basic TCM syndromes, occupied 41.8%. The other ones are qi-yin deficiency (30.23%), liver-kidney deficiency (13.95%), phlegm stasis (9.30%) and dampness- heat stagnation syndrome (4.65%). 3. There is not significant correlation between thyroid hormone levels and spleen-kidney deficiency syndrome. 4. There is significant difference in scores of MMSE which is different from scores of ADL between MCI group and control group. 5. Analysis on the general clinical features of MCI suggests that ,the common coures of MCI patients is over 1-2years; there are not obvious predisposing and mitigating factors as well as family history of most MCI patients. Conclusion: The results suggest that low TSH level maybe a risk factor for the occurrence of MCI. The most common TCM syndrome of MCI is spleen-kidney deficiency, manifesting that spleen and kidney deficiency plays an important role in the pathogenesis of MCI. There is not significant correlation between thyroid hormone levels and spleen-kidney deficiency syndrome.
