      目的 探讨参附强心合剂对心衰大鼠心肌能量代谢 AMPK-PGC-1α通路的调控及其作用机制。方法 采用腹主动脉缩窄法制作大鼠心衰模型,经4周造模成功后将造模组大鼠随机分为:模型组,参附强心合剂高、中、低剂量组,曲美他嗪组,予药物溶液灌胃4周后,取腹主动脉血及左心室组织,用 ELISA 法测血清中BNP、LDH、FFA含量,western blot 法对心室组织中AMPK、p-AMPK、PGC-1α进行半定量分析。结果 (1)各中药组血清BNP、LDH、FFA较模型组显著降低(P<0.05),其中,中药高剂量组BNP、FFA水平分别为(265.74±19.89)、(482.64±23.89),与曲美他嗪组无显著差异(P>0.05);中药高、中剂量组LDH水平分别为(295.25±29.34)、(317.83±25.99),与曲美他嗪组相当(P>0.05);(2)各组大鼠心肌组织中AMPK 蛋白表达水平未见显著差异(P>0.05);中药高剂量组p-AMPK蛋白表达(1.128±0.085)与曲美他嗪组均明显增加(P<0.01),两者之间未见明显差异(P>0.05);各用药组PGC-1α蛋白表达均明显增加(P<0.05),其中中药高剂量组(1.146±0.187)、中剂量组(1.138±0.188)、低剂量组(1.415±0.330),且中药各剂量组PGC-1α蛋白表达与曲美他嗪组之间无显著差异(P>0.05)。结论 1、参附强心合剂可以降低心衰大鼠的血清BNP、FFA、LDH水平从而减轻心衰。2、参附强心合剂可能通过激活AMPK-PGC-1α信号通路,提高p-AMPK、PGC-1α的表达以调节脂肪酸氧化代谢及葡萄糖转运,从而改善心衰大鼠心肌能量代谢,从而起到治疗心衰的作用。

      Objective To investigate the effects of Shenfu Qiangxin Decoction on AMPK-PGC-1α pathway of myocardial energy metabolism in rats with heart failure. Method The rat model of heart failure was made by coarctation of the abdominal aorta.After 4 weeks, the rats were randomly divided into the model group, high, medium, low dose of Shenfu Qiangxin Decoction group, trimetazidine group,and given by drug solution for 4weeks. The abdominal aortic blood and left ventricular tissue were collected at the eighth week. The ELISA method was used to detect the contents of BNP,LDH and FFA in serum. The western blot method was used forthe semi quantitative analysis of AMPK, p-AMPK and PGC-1α in ventricular tissue. Results (1)Compared with the model group, the serum BNP, LDH, FFA in the Chinese medicine groups, and trimetazidine group decreased.(P<0.05);(2) The expression level of AMPK protein in the myocardial tissue of rats in each group had no significant difference(P>0.05). Compared with the model group, high dose of Chinese medicine group(1.128±0.085) and trimetazidine group were significantly increased(P<0.01). Compared with the model group, the expression of protein in each drug group (high 1.146±0.187、middle 1.138±0.188、low 1.415±0.330)were significantly increased(P<0.05). Each dose of Shenfu Qiangxin Decoction group and trimetazidine group were significantly increased(P<0.01). Conclusion 1、Shenfu Qiangxin Decoction can reduce the level of serum BNP, FFA, LDH in rats with heart failure induced by coarctation of the abdominal aorta. 2、Shenfu Qiangxin Decoction may increase the expression of p-AMPK and PGC-1α to regulate fatty acid metabolism and glucose transport through the activation of the signal pathway of AMPK-PGC-1α, so as to improve the myocardial energy metabolism in rats with heart failure, and thus play a role in the treatment of heart failure.
