| 目的 探讨2型糖尿病患者视网膜病变与血清维生素D水平的相关性。方法 选取178例住院２型糖尿病患者,其中80例无视网膜病变(A组), 52例伴有非增殖性视网膜病变(B组), 46例伴增殖性视网膜病变(C组),并选取50例健康体检者为对照者(NC组)。记录各组一般临床资料,并测定各组空腹血糖(FBG) 、糖化血红蛋白(HbA1c)、25-(OH)D3、钙(Ca)、甲状旁腺激素(PTH),分析视网膜病变与血清25-(OH)D3的相关性及可能机制。结果 A、B、C三组血清25-(OH)D3水平均明显低于NC组,且随着视网膜病变进展而逐步下降 (均P <0.01)。Logistic逐步回归分析表明,2型糖尿病患者视网膜病变程度与血清25-(OH)D3水平负相关,与糖化血红蛋白水平正相关(均P <0.01)。结论 25-(OH)D3低下是2型糖尿病视网膜病变发生、进展的独立危险因素,提示在积极控制血糖的同时,适当给予维生素D制剂可能有益于防治糖尿病视网膜病变。 |

| Objective To investigate the relationship between diabetic retinopathy (DR) and serum 25-(OH)D3 in type 2 diabetic patients. Methods 178 diabetic inpatients were selected, in which 80 patients without DR(group A),52 patients with simple DR(group B)and 46 patients with proliferative DR(group C).Meanwhile,50 healthy people were selected as normal control(group NC).Fasting blood glucose(FBG), glycoslated hemoglobinA1c(HbA1c), serum 25-(OH)D3, calcium(Ca),parathyroid hormone(PTH) were tested,and relationship between DR and 25-(OH)D3 was analysed. Results The 25-(OH)D3 levels of groupA 、B、C were obviously lower than that of group NC(P<0.01),and with the aggravation of DR ,it gradually declined(P<0.01). Multivariate logistic regression showed that DR and 25-(OH)D3 had significantly negative correlation ,while DR and HbA1c had significantly positive correlation(both P<0.01). Conclusion The lower level of 25-(OH)D3 was the independent risk factor of DR in type 2 diabetic patients. Proper supplement of vitD and may be beneficial for retinopathy in type 2 diabetic patients. |
