      Abstract:Objective: To observe the effects of ulinastatin on the intestinal mucosal barrier function in rats with Ventilator-Associated pneumonia,and investigate its mechanism.Methods:A total of 100 male clean SD rats were divided into 5 groups.Ulinastatin group: VAP models, received 2ml Ulinastatin(100000U/kg) by intravenous injection every day;glutamine group:VAP models, received 2ml Glutamine(0.75g/kg)by by intravenous injection every day; Ulinastatin combined with glutamine group:VAP models, received 1ml Ulinastatin and 1ml Glutamine by intravenous injection every day; placebo group:VAP models, received 2ml saline by intravenous injection every day;blank control group: rats without special treatment,received 2ml saline by intravenous injection every day.Urine was collected for fifth days after surgery,the permeability of intestinal mucosa was evaluated by using lactulose/mannitol.Rats were killed at sixth days and mesenteric lymph nodes were cultured.Results:Compared with the blank control group on the intestinal permeability ,L/M in placebo group was increased obviously (P<0.005), while the other three groups were similar to those in the blank control group.L/M in glutamine group was was smaller than that of ulinastinal group,but there was no significant difference.In terms of bacterial translocation, the placebo group was significantly higher than the blank control group (P<0.005), the positive rate of the other three groups is lower than the blank control group, culture was mainly Escherichia coli based intestinal parasitic bacteria.Conclusions:The model of ventilator-associated pneumonia caused the destruction of intestinal mucosal structure , permeability increase and bacterial translocation in rats. Intestinal use of ulinastatin can reduce intestinal mucosal permeability, protect intestinal mucosa structure and reduce bacterial translocation. The protective effect on intestinal mucosal structure was not more pronounced by combined using of glutamine and ulinastatin .
