| [摘要] 目的 系统评价益气养阴活血方治疗早期糖尿病肾病（DKD）的临床疗效及安全性。 方法 检索中国期刊全文数据库、万方数据库、维普中文科技期刊全文数据库、中国生物医学文献数据库收录的关于益气养阴活血方治疗早期DKD的随机对照试验，纳入的文献进行方法学质量评价，并采用Rev Man 5.3 软件进行Meta分析，评价指标包括临床疗效、Scr、BUN、UAER及安全性评价。 结果 纳入符合标准文献18篇，涉及患者1429例，Meta分析结果显示：应用益气养阴活血方治疗早期DKD的临床总有效率明显高于常规西医治疗的对照组（OR=3.59，95%CI[2.72,4.75]）；应用益气养阴活血方或其联合ACEI/ARB类药物较单用ACEI/ARB类药物在降低早期DKD患者的Scr（SMD=-20.46, 95%CI[-32.10，-8.82]）和UAER（SMD=-37.18, 95%CI[-66.51，-7.86]）方面有更好的作用，但益气养阴活血方与常规西医治疗在降低BUN方面无显著差异（P＞0.05）。在不良反应方面，1/3的研究明确报告没有不良反应发生。 结论 益气养阴活血方治疗早期DKD在提高临床疗效、降低UAER和Scr及安全性方面具有优势，但尚需更多高质量研究做进一步分析论证。 |

| ABSTRACT Objective To evaluate the effect and safety of Yiqi yangyin huoxue recipe in treating early diabetic kidney disease(DKD) systematically. Methods Randomized controlled trials about Yiqi yangyin huoxue recipe treating early DKD were reviewed in China National Knowledge Infrastructure, VIP Database, Wanfang Database, Chinese Bio Medical Database. The methodology quality of included papers was evaluated. Rev Man 5.3 software was adopted to make Meta analysis. The evaluation indexes included clinical effect evaluation, Scr, BUN, UAER and safety evaluation. Results 18 papers met the standard were included, about 1429 patients. Meta analysis manifested that the total clinical efficacy in Yiqi yangyin huoxue recipe group was obviously superior to that in the controlled group(OR=3.59，95%CI[2.72,4.75]); Yiqi yangyin huoxue recipe or it combined with ACEI/ARB could decreased the level of Scr(SMD=-20.46, 95%CI[-32.10，-8.82]) and UAER(SMD=-37.18, 95%CI[-66.51，-7.86]) better than ACEI/ARB drugs single-used; it has no significant difference in decreasing BUN with controlled group(P＞0.05). One of third of papers showed no adverse reactions and other safety problems occurred in the process of study. Conclusion Yiqi yangyin huoxue recipe in treating early DKD had advantages in improving clinical efficacy, decreasing Scr and UAER, but more high-quality researches were needed to make further analysis and demonstration. |
