      目的 通过饮食及运动干预,评价其对改善代谢综合征(metabolic syndrome, MS)儿童左室结构及功能的有效性。方法 收集来我院儿科门诊确诊≥10岁的MS儿童21例临床资料腹围、体重指数(BMI)、血糖、血脂等资料,经超声心动图(UCG)获取室间隔舒张末期厚度(IVSD)、左心室舒张末期内径(LVEDD)、左室后壁舒张末期厚度(LVPWD)及左室心肌质量(LVM)以评估左室结构,以二尖瓣舒张早期峰值流速(E峰)/舒张晚期峰值流速(A峰)(E/A峰比)及左室射血分数(LVEF)评价左室功能,并测定空腹血糖(FPG)、口服葡萄糖耐量实验2h血糖(OGTT)、高密度脂蛋白胆固醇(HDL-C)、非高密度脂蛋白胆固醇(non-HDL-C)及甘油三酯(TG)。予饮食及运动疗法2年后复查UCG及上述血液指标,评估左室结构及功能是否改善。结果 饮食及运动干预后LVSD、LVDD、LVPWD与LVM明显低于干预前(P<0.001),E/A峰比明显高于干预前(P<0.001),干预前后FPG、OGTT血糖、HDL-C、non-HDL-C及TG比较差异有统计学意义(P<0.01)。 结论 饮食与运动干预可改善MS儿童左室结构及功能,开展儿童MS早期干预势在必行。

      【Objective】 This study evaluated the effects of diet and exercise intervention on left ventricular geometry and function in children with metabolic syndrome (MS) children. 【Methods】 21 pediatric subjects (age:10-16 years) with MS were enrolled, and clinical characteristics such as abdominal perimeter, body mass index, blood glucose, blood pressure were collected. The echocardiography (UCG) parameters of interventricular septum diastolic thickness (IVSD), left ventricular posterior end-diastolic diameter (LVEDD), left ventricular posterior wall thickness (LVPWD) and left ventricular mass (LVM) were measured to evaluate left ventricular structure. The Mitral E- to A-wave (E/A ratio) and left ventricular ejection fraction (LVEF) were calculated to evaluate left ventricular function. Fasting plasma glucose(FPG),2h blood glucose post oral glucose tolerance test (OGTT), high density lipoprotein (HDL), non-HDL and triglyceride(TG) were determined. Besides blood parameters reviewed, those parameters of UCG were recalculated to confirm improvement left ventricular geometry and function following two-year diet and exercise intervention. 【Results】 Compared with MS children pre-diet and exercise intervention, MS children post-diet and exercise intervention were characterized with thinner LVSD and LVPWD, reduced LVEDD and, decreased LVM and E/A ratio (P<0.001). Significant difference of FPG, 2h blood glucose after OGTT, HDL, non-HDL and TG were found between pre- and post-diet and exercise intervention.【Conclusion】 The results suggest that diet and exercise intervention could be an effective strategy for significant improvement in left ventricular geometry and function of MS children. It is very necessary to prevent MS with early diet and exercise intervention.
