| 摘要 目的 比较经皮椎体后凸成形术（PKP）分别联合密固达或补肾壮骨汤治疗老年骨质疏松性椎体压缩骨折（OVCF）的临床效果。方法 共纳入120例老年OVCF患者随机分为对照组和观察组各60例，均采用PKP，对照组术后采用密固达（唑来膦酸5 mg）静脉滴注，观察组采用补肾壮骨汤，随访1年，比较术后1月、3月和6月疼痛模拟视觉评分（VAS）、椎体前缘和中部高度，骨折愈合率、再骨折发生率和药物不良反应。结果 两组随访VAS值均逐渐降低，术后1月和3月VAS值比较无差异，但观察组6月VAS值明显低于对照组，差异有统计学意义（P＜0.05）。两组术后1月椎体前缘和中部高度均较术前增加，术后1月和3月两组比较无差异，但观察组6月椎体前缘和中部高度明显大于对照组，差异有统计学意义（P＜0.05）。 随访两组骨折愈合率比较无差异，但观察组再骨折发生率低于对照组（P＜0.05）。两组均未发生明显不良反应。结论 PKP联合密固达或补肾壮骨汤治疗老年OVCF具有较好的安全性和有效性，补肾壮骨汤远期疗效优于密固达。 |

| Abstract Objective to study the clinical effects of percutaneous kyphoplasty(PKP) combined with Zoledronic acid or Bushen Zhuanggu Decoction for elderly osteoporotic vertebral compressive fractures(OVCF).Method A total of 120 consecutives were enrolled and divided randomly into control and observation groups,each of 60 cases,all patients received PKP,then the patients in control group got Zoledronic acid 5 mg ivgtt,and they in observation group adopted Bushen Zhuanggu Decoction;to compare differences of visual analogue scale(VAS)after one,three and six months, anterior and middle vertebral heights, fracture healing rate, refracture rate and adverse drug reaction during one year follow-up.Results The VAS values in the two groups were gradually lower after treatment,they after one and three months in the two groups were no statistical difference,while it after six months in observation group was significantly lower than control group(P＜0.05).The same to anterior and middle vertebral heights(P＜0.05).The fracture healing rate in two groups was no statistical difference,while refracture rate in observation group was significantly lower(P＜0.05).And there were no complications in the two groups.Conclusion It is safe and effective of PKP combined with Zoledronic acid or Bushen Zhuanggu Decoction for elderly OVCF,and the long-term effect of Bushen Zhuanggu Decoction may be better than Zoledronic acid. |
