| 目的 探讨肾透明细胞癌的多层螺旋CT表现及影像学表现的病理机制。方法 收集经病理证实的肾透明细胞癌41例,分析其CT表现,并与病理学结果进行对照研究。结果 41例均为单发,其中右肾l3例,左肾28例。CT平扫：18例(43.9%)表现低密度,20例(48.8%)表现为等-低混杂密度,3例(7.3%)表现为稍高密度；38例(92.7%)可见囊变坏死区,23例(56.1%)可见假包膜,5例(12.2%)出现斑点状、颗粒状钙化；CT增强扫描：38例(92.7%)肿瘤实性部分皮髓质期呈明显不均匀强化,实质期强化程度明显减弱,排泄期强化程度进一步下降,呈典型“快进快退”强化模式；3例(7.3%)肿瘤轻中度强化、延迟强化。结论 肾透明细胞癌具有典型的CT表现,且这些表现可从病理学角度进行解释,影像学检查可为临床正确诊断及治疗提供依据。 |

| ABSTRACT: Objective To explore the multi-slice CT features and the pathomechanism of CT features of renal clear cell carcinoma. Methods Forty-one cases of renal clear cell carcinomas proved by pathology were collected, and the pathomechanism of CT features were retrospectively analyzed. Results All of the 41 cases were solitary, 13 cases on the right side, 28 cases on the left side. 18 tumors demonstrated low density，20 tumors demonstrated iso-density and low-density,and 3 tumors demonstrated slightly higher density.38 tumors had cystic necrotic area，23 tumors had pseudo-capsule，5 tumors had punctate and granular calcification.The real parts of 38 tumors showed typical“fast forward and fast rewind”enhancement pattern ,which was obvious heterogeneous enhancement in the cortical phase of enhancement scanning,the degree of enhancement was obviously decreased in the parenchymal phase,and declined further in the excretion phase.3 cases（7.3%）showed mild to moderate enhancement and delayed enhancement.Conclusion The CT features of renal clear cell carcinomas were typical,and the CT features had corresponding pathomechanism. The imaging examination could provide a basis for clinical diagnosis and treatment. |
