| 目的：探讨基质金属蛋白酶-9、血小板参数在不同类型脑梗死患者中表达及对预后的预测价值。方法：选取IWSI患者126例,CWSI患者70例以及70里健康人员作为对照,分析各组基质金属蛋白酶-9、血小板参数表达情况。结果：CWSI组、IWSI组TC、TG、hs-CRP、MMP-9、MPV及PDW均显著高于对照组(t值4.697~19.527,P<0.05)；IWSI组大脑中动脉狭窄比例、MMP-9、PLT及MPV均显著高于CWSI组(χ2=8.693,P=0.009；t=13.214,6.183,4.832,P=0.000,0.019,0.037)。Logistic回归分析显示,MMP-9(OR：1.893,95%CI：1.117~3.896)为CWSI的独立危险因素,MMP-9(OR：2.834,95%CI：1.634~5.717)、MPV(OR：1.939,95%CI：1.024~3.326)为IWSI独立危险因素。ROC曲线显示,MPV+ MMP-9预测IWSI发生高于MPV、MMP-9单一诊断价值。IWSI组预后不良患者MMP-9、MPV均显著高于CWSI组(t=9.046,6.337,P=0.004,0.021)。结论：MMP-9、MPV参与了IWSI大脑中动脉狭窄以及血流动力学异常过程,对IWSI预后不良有一定的预测作用。#$NL关键词：分水岭脑梗死；基质金属蛋白酶-9；血小板参数；预后 |

| Objective: To investigate the expression of matrix metalloproteinase -9 and platelet parameters in patients with different types of cerebral infarction and its prognostic value. Methods: 126 cases of IWSI patients, CWSI patients, and 70 healthy persons were selected as the control group. The expression of matrix metalloproteinase -9 and platelet parameters in each group of patients with were analyzed. Results: CWSI group, IWSI group, TC, TG, hs-CRP, MMP-9, MPV and PDW were significantly higher than control group (t = 4.697~19.527, P<0.05); group IWSI middle cerebral artery stenosis ratio, MMP-9, PLT and MPV were significantly higher than that of group CWSI (2=8.693, P=0.009; t=13.214,6.183,4.832, P=, 0.000,0.019,0.037). Logistic regression analysis that MMP-9(OR:1.893, 95%CI: 1.117~3.896) were independent risk factors of CWSI, MMP-9 (OR:2.834, 95%CI:1.634~5.717), MPV(OR:1.939, 95%CI:1.024~3.326) showed that the independent risk factors of IWSI. ROC curve MPV MMP-9 the prediction of IWSI is higher than MPV, MMP-9 single diagnostic value of poor prognosis in patients with MMP-9.IWSI group, MPV was significantly higher than that of group CWSI (t=9.046,6.337, P=0.004,0.021). Conclusion: MMP-9 and MPV are involved in the process of IWSI middle cerebral artery stenosis and hemodynamic abnormalities, which may be a predictor of poor prognosis of IWSI. |
