| 目的 探讨关节镜下治疗肩袖撕裂伴肩关节粘连的临床效果。方法 2012年2月~2015年7月,对32例肩袖撕裂伴肩关节粘连在肩关节镜下进行松结以及5.0 Twinfix锚钉单排修补术,术后均予康复指导。术后随访时间为随访6-24月,平均13.4月。比较术前与术后VAS评分,以及比较术前,术后6个月,12个月UCLA、ASES评分,评估患者肩关节功能恢复情况及手术疗效。 结果 患者术后VAS评分明显优于术前(p＜0.05),表明患者术后疼痛症状较术前明显好转；术后6个月与术前相比ASES评分(t=40.180Sp=0.00)、UCLA评分(t=54.884,p＝0.00),术后12个月与术前相比ASES评分(t=42.226p=0.00)、UCLA评分(t=51.338Sp=0.00),均有统计学意义,说明患者术后肩关节功能明显好转。术后12月UCLA评分为(31.281±2.543),优9,良21例,差2例,优良率93.75%。 结论 通过肩关节镜治疗肩袖撕裂伴肩关节粘连临床疗效确切,值得推广。 |

| Objective To investigate the clinical effect ofSSarthroscopic for treatment of rotator cuff tear combined with joint adhesion. Methods From February 2012 to July 2015,32 patients with rotator cuff tear associated with shoulder joint adhesion treated by arthroscopy,which through capsule release and single-row(5.0 Twinfix anchor),then given postoperative rehabilitation guide. All the patients postoperative follow-up for 6 to 24 months, an average of 13.4 months.Visual analogue scale(VAS) were compared among preoperative and postoperative,University of California at Los Angeles(UCLA) and American Shoulder and Elbow Surgeons(ASES) were compared among preoperative and postoperative 6 months, 12 months,to evaluate the conditions of shoulder joint functional recovery. Result As compared to preoperative level,the postoperative VAS scores significantly lower(p<0.05),showed that the pain were improved obviously postoperative; At postoperative 6 months,12 months,the UCLA and ASES scores significantly increased as compared to preoperation (p<0.05),showed that the postoperative improvement of shoulder joint functional recovery obviously.According to the postoperative 12 months UCLA scores were(31.281±2.543),9 cases were excellent,21 cases were good and 2 cases were fair:the excellent and good rate was 93.75%.Conclusion Treated by arthroscopic could bring about a striking effect for whose shoulder rotator cuff tear combined with shoulder joint adhesion, which is worth to be popularized. |
