      目的:探讨咪达唑仑对重症外伤患者的脑代谢的影响研究.方法:选取2014年12月至2015年12月于本院重症医学科以外伤导致的严重脑损伤为诊断而入院的患者80例,随机分为两组:对照组40例,予临床常规基础治疗外,加用枸缘酸芬太尼注射液、丙泊酚注射液和右美托咪定治疗;实验组40例,给予常规基础治疗,枸缘酸芬太尼注射液、丙泊酚注射液和咪达唑仑注射液治疗.观察两组患者治疗前后心率、平均动脉压、PaO2、PjvO2、SjVO2、Da-jvO2、CERO2、血清S-100β蛋白和NSE水平变化.结果:与治疗前比较,两组患者治疗后的心率、平均动脉压、Da-jvO2、CERO2、血清S-100β蛋白以及NSE水平均明显降低,PaO2、PjvO2和SjVO2水平均有升高;治疗后与对照组比较,实验组的心率、平均动脉压、Da-jvO2、CERO2、血清S-100β蛋白以及NSE水平均明显低于对照组,PaO2、PjvO2和SjVO2水平明显高于对照组,以上结果差异均有统计学意义(P<0.05).结论:咪达唑仑能有效降低了重症外伤患者脑代谢水平,值得临床加以应用和推广.

      Objective: To investigate the effect of Midazolam midazolam in the treatment of Severe trauma in Cerebral metabolism.Methods: 80 cases of severe brain trauma from 2014.12 to2015.12 in our hospital were selected and randomly divided into 2 groups, 40 cases in the control group received clinical conventional treatment, fentanyl citrate injection, Propofol Injection and dexmedetomidine, the experiment group received more with Fentanyl citrate injection, Propofol Injection, Midazolam Injection treatment, The heart rate, mean arterial pressure, PaO2, PjvO2, SjVO2, Da-jvO2, CERO2, levels of serum S-100β and NSE were compared before and after treatment in two groups. Results: Compared with before treatment, levels of heart rate, mean arterial pressure, Da-jvO2, serum CERO2, S-100 and NSE protein in two groups of patients after treatment decreased, and levels of PaO2, PjvO2 and SjVO2 increased(P<0.05), compared with the control group, levels of heart rate, mean arterial pressure, Da-jvO2, serum CERO2, S-100 and NSE protein in the experiment group were lower, levels of PaO2, PjvO2 and SjVO2 were higher, the differences were statistically significant (P<0.05). Conclusion: Midazolam can effectively reduce the level of cerebral metabolism in patients with severe traumatic injury, and it is worth to be applied and popularized in clinic.
