      【】 目的 观察远端万向锁定接骨板不植骨治疗桡骨远端粉碎性骨折的临床疗效。方法 2011年12月至2015年4月我院骨伤科收治的采用远端万向锁定接骨板不植骨治疗的A3、C2、C3型老年不稳定性桡骨远端骨折46例患者,所有患者为均为桡骨短缩(和)或关节面塌陷的闭合性骨折。术前术后均评估掌倾角、尺偏角、关节面畸形情况。结果 所有患者术后获11~57个月(平均30.6个月)随访。x线片示骨折愈合时间为2.5~4个月,平均2.7个月。影像学指标明显改善,采用Gartland—Werley功能评分评定疗效:优22例,良20例,可4例,优良率为91.3%。无伤口感染患者或桡动脉、正中神经受损伤患者。结论 本组病人单独采用远端万向锁定接骨板不植骨治疗此类桡骨远端骨折可获得骨性愈合及满意的临床疗效。

      【】Objective To observe the clinical results of the application of volar variable angle locking plate without bone grafting in the treatment of unstable distal radial fractures of old people.Methods From December 2011 to April 2015,46 cases of type A3,type C2 and type C3 comminuted distal radial fractures were treated with variable angle locking plate locking plate without any bone grafting.All the cases were closed fractures with radial shortening or articular surface collapse.Volar tilt angle,radial inclination angle,extent of joint surface step deformity were measured both pre and post-operatively.Results All the patients were followed-up for 11 to 57 months with an average of 30.6 months.Bone union was noted in x-ray in 2.5-4months(average 2.7 months).Radiographic parameters were significantly improved.The clinical results were evaluated using the Gartland—Werley wrist score and rated as excellent and good is 91.3%(22 patients are excellent,20 are good ,4 are fair).Conclusion Variable angle locking plate without bone grafting can achieve bony union and satisfied clinal outcomes in treating type A3.C2 and C3 comminuted distal radial fractures.
