| 目的：为了进一步提高临床对甲状腺癌伴颈部淋巴结肿大的诊断效果,分析研究彩色超声在诊疗过程中的应用价值。方法：将2014年1月-至2016年1月我院治疗的339例甲状腺癌(病理明确)患者作为研究对象,入组患者均对颈部肿大淋巴结进行彩色超声检查,明确淋巴结的一般状况(大小、形状、边界、厚度、是否融合、血流信号等)并同时和病理活检结果进行对比。结果：研究数据显示,入组的339例甲状腺癌患者中共发现213例出现颈部淋巴结转移,其中术前彩色超声确诊204例,经病理确诊180例,彩色超声检查准确率为84.51%。甲状腺癌患者应用彩色超声对颈部肿大淋巴结进行检查,对皮质变厚、淋巴门发生偏移、边界模糊不清、淋巴结相互融合的诊断灵敏度及特异度分别超过90%和60%。结论：甲状腺癌伴颈部淋巴结肿大患者应用彩色超声进行检查,简单方便且有效,准确率和特异性较高,有助于临床术前评估患者病情,值得推广和应用。 |

| objective: In order to further improve the clinical diagnosis of thyroid carcinoma with cervical lymph node enlargement, the application value of color ultrasound in the diagnosis and treatment process is analyzed and studied. Method：In January 2014 - January 2016 339 cases of thyroid cancer patients treated in our hospital as the research object, Color ultrasonography was performed in the patients with cervical lymph nodes, the general state of the lymph nodes (size, shape, boundary, thickness, whether the fusion, blood flow signals, etc.) and the same time and pathological biopsy results were compared. Result：Research data display, In 339 cases of thyroid cancer, 213 cases of cervical lymph node metastasis were found in the patients with thyroid cancer, Preoperative color Doppler ultrasound diagnosis of 204 cases, 180 cases were confirmed by pathological diagnosis, color ultrasound examination accuracy rate was 84.51%. Application of color Doppler ultrasound in the diagnosis of cervical lymph nodes in patients with thyroid cancer, The diagnostic sensitivity and specificity of cortical thickening, lymph node migration, blurred boundary, and lymph nodes were more than 90% and 60% respectively. Conclusion: Application of color Doppler ultrasonography in patients with thyroid carcinoma and cervical lymph node enlargement, simple and convenient and effective, high accuracy and specificity, it is helpful to evaluate the patient"s condition before clinical operation, and it is worth popularizing and applying. |
