| 目的 研究骨水泥联合液态人工骨行胸腰椎后凸成型术对邻椎应力性骨折的预防作用。 方法 选取2011年1月-2016年1月行胸腰椎后凸成型术的骨质疏松患者136例,观察每个患者术中出血量、手术时间、术后住院时间、疼痛、影像学评价椎体平均高度及后凸Cobb角,术后随访6-24月关注伤椎邻近椎体的应力性骨折发生情况。结果 此种手术方法术中出血量为21.4±3.54 ml,均未输血,手术时间 96±18min,术后住院天数7.63±1.76d,术后椎体平均高度较术前平均高度增加2.18±0.46mm,术后后凸Cobb角较术前后凸Cobb角减少了2.55±0.27°,术后门诊随访16±7.76月,疼痛评分由入院时的7.65±0.76降为治疗后1.02±1.52,满意率为100%(p<0.01),骨水泥渗漏率为6.25%,邻近椎体骨折发生率占总椎体数5.11%。 结论 骨水泥联合液态人工骨行胸腰椎后凸成型术不仅具有手术出血少、手术时间短及并发症少等优点,而且对邻椎应力性骨折具有积极的预防作用。 |

| Objective: We are aimed to study about Prevention of stress fracture of adjacent via applying for bone cement combines with liquid artificial bone to PKP in thoracolumbar fractures.Methods:Form January 2011 to January 2016,136 patients with osteoporosis apply for bone cement combines with liquid artificial bone to PKP,observe patients blood loss、time during operation、post operation hospitalization uration、pain、iconographyevaluating the average height and convex Cobb angle after the vertebral bodies,postoperative follow-up of 6 to 24 months pays close attention to on injury vertebral stress fracture of adjacent vertebral body.Results：This kind of surgery methods intraoperative blood loss is 21.4±3.54ml no blood transfusion,time of operation is 96±18min,time of postoperative hospital is 7.63±1.76d,average height of postoperative vertebral body compared with preoperative increases 2.18±0.46mm,convex Cobb angle after the vertebral bodies of postoperative vertebral body compared with preoperative decreases 2.55±0.27°,postoperative outpatient follow-up 16±7.76 months,pain score decreases from being admitted to hospital 7.65±0.76 to hospital for posttreatment 1.02±1.52,satisfaction rate is 100%(p<0.01),bone cement leakage rate is 6.25%, the incidence of adjacent vertebral fractures account for 5.11% of the total number of vertebral body.Conclusion：Bone cement combines with liquid artificial bone applying for PKP not only has the operation less bleeding, shorter operation time and less complications, but also stress of adjacent vertebral fracture in a positive role in prevention. |
