| 目的 探讨应用克氏针动态牵引支架结合有限内固定治疗手指近指间关节Pilon骨折的近期疗效。方法 对28例患者31指应用自制克氏针动态牵引支架结合有限内固定治疗闭合近指间关节Pilon骨折,定期复查X线片,同时指导功能锻炼,评定关节主动活动范围,测量健指与患指术后近指间关节间隙大小并做对比。 结果 术后所有患者均获得随访,骨折愈合良好,按照手指关节总活动度系统评价标准来评定,优 15良12 差1,优良率为87.1%。患指术后近指间关节间隙0.69±0.14mm,其余健指0.74±0.13mm,两者差异无统计学意义(P>0.05)。 结论 自制克氏针动态牵引支架结合有限内固定用于治疗近指间关节Pilon骨折手术方式效果肯定,能进行早期功能锻炼,避免关节间隙狭窄,促进恢复关节功能。 |

| Objective To evaluate the clinical short-term effects and safety of dynamic external fixtor combining with limited internal fixation on the treatment of close Pilon fractures of the proximal interphalangeal joint. Method: Totally 28 cases of 31 fingers patients with close interphalangeal joint Pilon fracture were treated by dynamic external fixtor combining with limited internal fixation, regular reexamination of X ray film, guided functional exercise, assessed the active range of joints. It is contrasted that the health and postoperative proximal interphalangeal joint space have measured. Results All patients were followed up. All the fractures healed uneventfully. Rated by the total active range of motion system,functions were excellent in 15 fingers,good in 12 fingers, poor in 1 finger,with an overall satisfactory rate of 87．1％. The measurement of postoperative proximal interphalangeal joint space is 0.69±0.14mm,the health is 0.74±0.13mm, there was no significant difference (P>0.05). Conclusion: Dynamic external fixtor combining with limited internal fixation is a reliable and effective operation method to treat Pilon fractures of the proximal interphalangeal joint .It allows early postoperative functional rehabilitation, avoids narrow joint space, and restores the joint function. |
